小剂量多巴胺联合米力农对顽固性心力衰竭的疗效及对心功能的影响

摘　　要：目的 对顽固性心力衰竭患者实施小剂量多巴胺与米力农联合治疗产生的应用效果作出分析与研究。方法 选取2019年7月-2020年12月本院收治的84例顽固性心力衰竭患者作为研究对象,以随机抽签的方式将患者分入常规组和治疗组,常规组42例患者运用常规方法治疗,治疗组42例患者运用常规方法治疗的同时,配合小剂量多巴胺联合米力农的方法进行治疗,针对两组临床诊治效果展开比较和分析。结果 与常规组患者治疗后心功能相关指标评定结果相比,治疗组左心室射血分数、6分钟步行距离显著更高,同时左室舒张末期内径显著更低(P<0.05);与常规组患者治疗后血清相关指标测定结果相比,治疗组肌酸激酶同工酶、肌酸激酶以及N-末端B型利钠肽前体水平均显著更低(P<0.05);与常规组患者临床治疗总有效率相比,治疗组显著更高(P<0.05);与常规组患者治疗后生活质量评分结果相比,治疗组精神状态、躯体功能、心理功能以及社会功能评分均明显更高(P<0.05)。结论 小剂量多巴胺与米力农联合治疗的方式能够有效促进顽固性心力衰竭患者诊治效果的提升,适合深入探究与普及。Objective To analyze and study effect of combined treatment of low dose dopamine and milrinone for patients with intractable heart failure.Methods The paper chose 84 patients with intractable heart failure from July 2019 to December 2020 as study subjects,and divided them into two groups randomly(including routine group and treatment group).42 cases in routine group were treated with routine method,while 42 cases in treatment group with the routine method.At the same time,they were treated with low-dose dopamine combined with milrinone.Clinical diagnosis and treatment effect between two groups was compared and analyzed.Results:comparison of evaluation results of indexes related to cardiac function after treatment showed,left ventricular ejection fraction and 6-minute walking distance in treatment group was significantly higher,and left ventricular end-diastolic diameter was significantly lower(P<0.05);levels of creatine kinase isoenzyme,creatine kinase and N-terminal B-type natriuretic peptide precursor in treatment group were significantly lower than control group(P<0.05);total effective rate of clinical treatment in treatment group was significantly higher than routine group(P<0.05);comparison of life quality scores in routine group after treatment showed,scores of mental state,physical function,psychological function and social function in treatment group were significantly higher(P<0.05).Conclusion Combined treatment of low-dose dopamine and milrinone can promote diagnosis and treatment of refractory heart failure patients effectively,which is suitable for indepth exploration and promotion.
