参续补骨汤联合温针灸对绝经后骨质疏松性腰背痛的镇痛效果及抗骨质疏松机制

摘　　要：目的 观察用参续补骨汤联合温针灸对绝经后骨质疏松性腰背痛的镇痛效果,并对抗骨质疏松机制进行初步探讨。方法 按照随机数表法将2020年2月至2021年2月于重庆市武隆区中医院就诊的112例绝经后骨质疏松性腰背痛患者分为对照组和观察组,每组各56例。对照组给予常规西药治疗,观察组在对照组基础上给予参续补骨汤联合温针灸治疗。比较两组的总有效率、视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)、骨密度(BMD),监测血清中骨骼生长相关指标[生长激素(GH)、胰岛素样生长因子-1(IGF-1)]、疼痛因子[前列腺素F2α(PGF2α)、P物质(SP)]水平。结果 观察组的总有效率明显高于对照组(P <0.05);治疗1周、2周、3个月后,与对照组比较,观察组的VAS评分均明显降低(P <0.05);治疗后,观察组的腰背功能、骨密度改善优于对照组,ODI评分显著低于对照组,BMD明显高于对照组(P <0.05),血清中骨骼生长相关指标GH、IGF-1含量显著高于对照组(P <0.05),疼痛相关指标PGF2α、SP含量显著低于对照组(P <0.05)。结论 参续补骨汤联合温针灸利于缓解绝经后骨质疏松性腰背痛患者的痛感,提高骨密度,其起效可能与促进血清GH、IGF-1分泌、减少PGF2α、SP的合成和分泌有关。Objective To observe the analgesic effect of Shenxu Bugu Decoction combined with warm acupuncture and moxibustion on postmenopausal osteoporotic low back pain,and preliminarily explore the anti-osteoporosis mechanism.Methods According to the random number table method,112 patients with postmenopausal osteoporotic low back pain who were treated in Chongqing Wulong District Hospital of Chinese Medicine from February 2020 to February 2021 were divided into the control group and the observation group,with 56 patients in each group.The control group was treated with conventional Western medicine,while the observation group received Shenxu Bugu Decoction combined with warm acupuncture and moxibustion on the basis of the same therapy received by the control group.The total effective rate,visual analogue scale(VAS)score,Oswestry disability index(ODI)score and bone mineral density(BMD)were compared between the two groups,and the serum bone growth related indicators including growth hormone(GH)and insulin-like growth factor-1(IGF-1)as well as pain factors including prostaglandin F2α(PGF2α)and substance P(SP)were monitored.Results The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).After 1 week,2 weeks and 3 months of treatment,the VAS score of pain in the observation group was significantly lower than that in the control group(P<0.05).After treatment,the improvements of waist and back function and bone mineral density(BMD)in the observation group were better than those in the control group;ODI score was significantly lower than that in the control group,BMD was significantly higher than that in the control group(P<0.05);the levels of serum bone growth related indicators,GH and IGF-1,were significantly higher than those in the control group(P<0.05);the levels of pain related indexes,PGF2αand SP,were significantly lower than those in the control group(P<0.05).Conclusion Shenxu Bugu Decoction combined with warm acupuncture and moxibustion is beneficial to allevi
