摘 要:目的探讨下肢康复机器人运动训练对心力衰竭(心衰)患者心功能改善的疗效。方法入选120例慢性心衰患者,通过随机数字表法分为常规组和实验组,每组各60例,同期选择心衰不愿行心肺康复治疗的患者60例作为对照组。观察三组患者左心射血分数(LVEF)、左室舒张末径(LVEDD)、左室收缩内径(LVESD)、N末端脑钠肽(NT-BNP)、MLWHFQ评分、6 min步行距离、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、无氧阈时摄氧量(VO_(2)AT)、峰值摄氧量(peakVO_(2))及峰值代谢当量(peakMET)及MACE事件发生率。结果治疗前,三组患者上述指标无差异。治疗1月后,与对照组相比,常规组、实验组的LVEF升高(P<0.05),LVEDD、LVESD缩小(P<0.05),NT-BNP降低(P<0.05),MLWHFQ评分降低(P<0.05),6 min步行距离增加(P<0.05);CRP、TNF-α、IL-6均降低(P<0.01);与对照组相比,常规组、实验组VO_(2)AT、peakVO_(2)、peakMET均升高(P<0.05);常规组、实验组的MACE事件发生率降低(P<0.05)。结论下肢康复机器人能较好改善慢性稳定型心力衰竭患者心功能。Objective To investigate the effect of lower limb rehabilitation robot exercise training on the improvement of cardiac function in patients with heart failure.Methods 120 patients with chronic heart failure were enrolled and divided into the conventional group and the experimental group with 60 patients in each group by random number table method.60 patients with heart failure unwilling to receive cardiopulmonary rehabilitation were enrolled as the control group.Left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),left ventricular contractile diameter(LVESD),amino-terminal brain natriuretic peptide(NT-BNP),MLWHFQ score,6 Min walking distance,C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),anaerobic threshold oxygen uptake(VO_(2)AT),peak oxygen uptake(peakVO_(2)),peak metabolic equivalent(peakMET)and MACE event incidence.Results Before treatment,there was no difference in the above indexes among the three groups.After 1 month of treatment,compared with the control group,LVEF was increased(P<0.05),LVEDD and LVESD were decreased(P<0.05),NT-BNP was decreased(P<0.05),MLWHFQ score was decreased(P<0.05),and 6-minute walking distance was increased(P<0.05).CRP,TNF-αand IL-6 were all decreased(P<0.01).Compared with control group,VO_(2)AT,peakVO_(2)and peakMET were increased in routine group and experimental group(P<0.05).The incidence of MACE events in conventional group and experimental group was decreased(P<0.05).Conclusions Lower limb rehabilitation robot can improve the cardiac function of patients with chronic stable heart failure.
