灸疗对类风湿关节炎患者的临床疗效及对炎性因子的影响

摘　　要：目的:观察灸疗联合甲氨蝶呤对类风湿关节炎(Rheumatoid Arthritis,RA)的疗效及其对患者血液炎性因子的影响。方法:将80例RA患者随机分为对照组和观察组,各40例。对照组给予甲氨蝶呤治疗,观察组在甲氨蝶呤治疗的基础上另外实施灸疗。经过12周的治疗,比较两组RA患者的临床症状、类风湿关节炎28项疾病活动评分(Disease Activity Score-28,DAS-28)评分和健康评估问卷(Health Assessment Questionnaire,HAQ)评分的改善情况,以及对炎性因子指标的影响。结果:治疗后,两组关节疼痛数、关节肿胀数、晨僵时间、DAS-28评分、HAQ评分、红细胞沉降率(Erythrocyte Sedimentation Rate,ESR)、C反应蛋白(C-reactive Protein,CRP)、白细胞介素(Interleukin,IL)-6、肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)等指标均较治疗前改善(P<0.05),且观察组较对照组有显著改善(P<0.01)。结论:灸疗可以改善RA患者的临床症状,降低DAS-28和HAQ评分,下调ESR、CRP、IL-6和TNF-α水平,对RA疗效显著。Objective:To investigate the efficacy of moxibustion plus methotrexate on rheumatoid arthritis(RA)and its effects on the levels of inflammatory factors.Methods:A total of 80 patients with RA were randomly divided into the control group and treatment group,40 cases in each group.The control group was treated with methotrexate,and the treatment group was treated by moxibustion plus methotrexate.After 12 weeks of treatment,the improvements of clinical symptoms,disease activity score-28(DAS28)scores,the health assessment questionnaire(HAQ)scores,and inflammatory factors levels of RA patients in both groups were assessed.Results:After 12 weeks treatment,the number of painful joints,number of swelling joints,and morning stiffness time were all ameliorated,meanwhile,the DAS-28 and HAQ scores,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),interleukin(IL)-6 and tumour necrosis factor-α(TNF-α)levels were all declined in both groups compared with those before treatment(P<0.05),and those in the treatment group was significantly better than the control group(P<0.01).Conclusion:Moxibustion can improve the clinical symptoms of RA patients,reduce DAS-28 and HAQ scores,reduce the levels of ESR,CRP,IL-6 and TNF-α,thus,moxibustion has a significant effect on RA.
