时间目标管理缩短急性缺血性脑卒中溶栓治疗时间窗的研究

摘　　要：目的探讨急性缺血性脑卒中患者急诊护理中采用时间目标管理方案对缩短溶栓治疗时间窗的作用。方法选择2019年7月—2022年6月进入卒中绿色通道的急性缺血性脑卒中患者80例作为研究对象。将患者按照入院先后顺序分为对照组(2019年7月—2020年12月)与试验组(2021年1月—2022年6月),各40例。两组分别进行常规管理、时间目标管理。比较两组开通静脉通道时间、CT完成时间、到达医院门口就诊至静脉溶栓的时间(door-to-needle time,DNT)、静脉溶栓有效率、护理管理满意度。结果试验组开通静脉通道时间、CT完成时间、溶栓DNT时间与对照组比较,差异有统计学意义(P<0.05)。试验组静脉溶栓有效率与对照组比较,差异有统计学意义(P<0.05)。试验组护理满意度与对照组比较,差异有统计学意义(P<0.05)。结论在急性缺血性脑卒中患者急诊护理中采用时间目标管理方案,能够缩短溶栓治疗时间窗,效果显著。Objective To explore the effect of time target management scheme on shortening the time window of thrombolytic therapy in emergency nursing of patients with acute ischemic stroke.Methods A total of 80 patients with acute ischemic stroke who entered the stroke green channel from July 2019 to June 2022 were selected as the study subjects.The patients were successively divided into the control group(from July 2019 to December 2020)and the test group(from January 2021 to June 2022),40 cases each.Routine management and time target management were carried out in the two groups respectively.The two groups were compared in terms of the time to open the venous channel,the time to complete CT,the time to visit the hospital and the time to intravenous thrombolysis(DNT),the efficiency of intravenous thrombolysis,and the satisfaction of nursing management.Results The time of venous passage opening,CT completion and thrombolytic DNT in the test group were statistically significant compared with those in the control group(P<0.05).The effective rate of intravenous thrombolysis in the test group was significantly higher than that in the control group(P<0.05).The nursing satisfaction of the test group was statistically significant compared with that of the control group(P<0.05).Conclusion The application of time target management scheme in emergency nursing of patients with acute ischemic stroke can shorten the time window of thrombolytic therapy with significant effect.
