刘克勤主任医师运用枳实消痞汤治疗脾胃病临床经验

摘　　要：枳实消痞汤出自李杲(号东垣)的《兰室秘藏》,该方组方严谨,寒热并用,可用于虚实寒热错杂之证。现代研究表明,枳实消痞汤能增强胃动力,促进胃排空,能调整胃肠激素的分泌,改善黏膜病理改变。刘克勤主任医师多年从事中医内科脾胃病及“治未病”方面的临床研究,擅长治疗脾胃病、身心疾病等疑难杂症,具有较高的专业能力及扎实的中西医理论基础,临床经验丰富,擅于灵活应用枳实消痞汤治疗脾虚气滞型的脾胃疾病,并取得了良好的疗效。导师临床治疗脾胃病时,常强调“以降为用、以通为补”的治疗方法,无论虚实皆可用通降之法。临证中脾胃病往往伴有肝气上逆,肝失疏泄,气郁日久,气机不畅,乘胃犯上。脾胃运化失常,脾虚中气不足,和降失调,肝木更易乘脾土。故导师认为,通降理论不应拘泥于脾胃,肝升肺降,相反相成;心散肾收,相辅相制;脾胃为使为市,五行灵动协调,则人体代谢正常。健脾醒脾,调节气机升降是导师治疗脾胃系疾病的重要思路。导师常以枳实消痞汤为主方进行加减化裁,治疗效果良好,现将导师相关临床经验,并附医案三则,以期为脾胃病的临床治疗提供更多参考。The Zhishi Xiaopi decoction(枳实消痞汤)is from LI Gao's Lanshi Micang(《兰室秘藏》),which is rigorously organized and used both cold and hot,which can be used to prove the mismatch of cold and heat.Modern studies have shown that the Zhishi Xiaopi decoction can enhance gastric motility,promote gastric emptying,adjust the secretion of gastrointestinal hormones,and improve mucosal pathological changes.Chief physician LIU Keqin has been engaged in clinical research on spleen and stomach diseases and preventive treatment in TCM internal medicine for many years,and is good at treating difficult and miscellaneous diseases such as spleen and stomach diseases,physical and mental diseases,with high professional ability and solid theoretical foundation of the integrated medicine,with rich clinical experience,and is good at flexibly applying the Zhishi Xiaopi decoction to treat spleen and stomach diseases with spleen deficiency and Qi stagnation type,and has achieved good results.When the instructor clinically treats spleen and stomach diseases.He often emphasizes the treatment method of using reduction as a use,and taking general as a supplement,regardless of whether it is virtual or real,the method of passing and lowering can be used.In the clinical evidence,spleen and stomach diseases are often accompanied by liver Qi(气)reversal,liver loss and leakage,Qi depression for a long time,poor Qi,and stomach offense.The spleen and stomach are dysfunctional,the spleen is deficient in Qi,and the descending disorder,and the liver wood is more likely to take advantage of the spleen soil.Therefore,the teacher believes that the theory of descent should not be limited to the spleen and stomach,the liver rises and the lungs fall,and the opposite is true.Scattered heart and kidney retraction,complementary to each other.The spleen and stomach are used to make the city,and the five elements are coordinated and the human metabolism is normal.Strengthening the spleen and awakening the spleen and regulating the lifting of the Qi m
