摘　　要：慢性肾脏病是各种原因导致的肾脏结构和功能障碍。中医内科中无“慢性肾脏病”病名,根据其水肿、少尿、无尿等临床表现以及其发病特点、病情演变经过和预后,可将其归属为中医学“水肿”“癃闭”“关格”“肾风”“肾劳”等范畴。由于慢性肾脏病病程缓慢、初期症状不明显,往往得不到患者的足够重视,等到就诊时病情经常已经非常严重了。患者临床表现为不同程度的浮肿、高血压、蛋白尿,还经常伴发严重的心血管疾病。因此,全社会都应该知晓慢性肾脏病危害,并大力提高防治力度。南京医科大学附属老年医院中医科孙世竹主任认为本病属“本虚标实”:脾肾亏虚为发病之本;水湿、湿热、血瘀为标;临证时注重健脾补肾、固本求源,而活血清利之法则要贯穿治疗始终。文章结合案例介绍孙主任治疗慢性肾脏病的经验,以期为临床提供参考。Chronic kidney disease is a disorder of renal structure and function caused by various reasons.Because the course of chronic kidney disease is slow,and the initial symptoms are not obvious,patients often do not pay enough attention to the disease,and the condition is often very serious when they go to outpatient to see a doctor.The clinical manifestations of the patients are different degrees of edema,hypertension,proteinuria,and often accompanied by serious cardiovascular disease.Therefore,the whole society should be aware of the harm of chronic kidney disease and improve the prevention and treatment efforts.Director SUN Shizu believes that the pathogeny of the disease is“fundamental deficiency and superficial excess”,that is,spleen and kidney both deficiency is the root cause of the disease,and wet,dampness-heat and blood stasis are exterior causes.Professor SUN Shizhu pays attention to strengthening the spleen and kidney to consolidate the foundation in clinical practice,and the method of invigorating blood circulation and promoting diuresis is used throughout whole course of the treatment.In this paper,Director SUN’s experience in treating chronic kidney disease is introduced through the case analysis,in order to provide a clinical reference.
