摘 要:目的探讨改良胃管置入长度体表测量方法在新生儿经口置管中的应用效果。方法选择2019年6月至2022年3月收治的98例接受持续胃肠减压术的新生儿,按照随机数字表法分为试验组和对照组,每组49例。对照组采用传统胃管置入长度测量方法,即测量鼻尖→耳垂→剑突的距离;试验组采用改良胃管置入长度测量方法,即测量鼻尖→耳垂→剑突与脐中点的距离。比较2组患儿胃管前端到位率、胃管置入长度、引流量、胃肠减压时间以及呕吐发生率。结果试验组胃管前端到位率为87.8%高于对照组的63.3%(P<0.05);试验组胃管置入长度长于对照组(P<0.05);试验组胃肠减压时间少于对照组(P<0.05);试验组呕吐发生率为6.1%低于对照组的20.4%(P<0.05)。术后1 d、2 d、3 d,试验组引流量高于对照组(P<0.05)。结论改良新生儿胃管置入长度体表测量方法可提高胃管到位率,改善胃肠减压效果,降低呕吐发生率。Objective To investigate the application efficacy of modified body surface measurement of orogastric tube insertion length in newborns.Methods A total of 98 newborns receiving orogastric tube insertion were randomly assigned to experimental group and control group,with 49 cases in each group.The orogastric tube insertion length in control group was conventionally measured by the length from the nose,earlobe to xiphoid.A modified measurement was adopted in experimental group by measuring the distance from the nose,earlobe and xiphoid to the midpoint of umbilicus.The rate of placing the orgastric tube,length of orogastric tube insertion,drainage volume,time to gastrointestinal decompression and incidence of vomitting between two groups were compared.Results The success rate of placing the orogastric tube in the experimental group was significantly higher than that in the control group(87.8%vs 63.3%,P<0.05).The time to gastrointesntinal decompression in the experimental group was significantly shorter than that in the control group(20.4%)(P<0.05).The incidence of vomiting in the experimental group(6.1%)was significantly lower than that in the control group(P<0.05).The drainage volume at 1,2 and 3 days postoperatively were significantly larger in the experimental group than those in the control group(all P<0.05).Conclusion Modified body surface measurement of orogastric tube insertion length in newborns can improve the correct placement of the orogastric tube and the effect of gastrointestinal decompression,and decrease the incidence of vomiting.
