老年慢性心力衰竭实施连续性无缝式社区医院一体管理模式效果及对生活质量的影响

摘　　要：目的探讨老年慢性心力衰竭实施连续性无缝式社区医院一体管理模式的效果及对患者生活质量的影响。方法选取2021年6月至2022年3月收治的老年慢性心力衰竭患者64例,按照随机数字表法分成观察组和对照组,每组32例。对照组采用常规模式,观察组采用连续性无缝式社区医院一体管理模式,对比2组干预效果及生活质量。结果观察组干预总有效率为93.75%显著高于对照组的75.00%,差异有统计学意义(P<0.05)。观察组患者HAMA、HAMD、PSQI评分低于对照组,差异有统计学意义(P<0.01)。干预前2组心功能无明显差异(P>0.05),干预后观察组LVEF、LVEDD、LAD、ΔPm各项心功能的恢复显著优于对照组,2组比较差异有统计学意义(P<0.05)。2组干预前生活质量评分比较无明显差异(P>0.05),干预后较干预前明显提高(P<0.05),与对照组比较,观察组评分更高(P<0.05)。结论采取连续性无缝式社区医院一体管理模式有利于提高老年慢性心力衰竭患者干预效果,改善患者心理及睡眠状态,提高患者生活质量,值得推广。Objective To explore the effect of continuous seamless community hospital integrated management mode and its influence on the quality of life in elderly patients with chronic heart failure(CHF).Methods A total of 64 elderly patients with CFH who were admitted to our hospital from June 2021 to March 2022 were randomized 1∶1 to conventional mode(the control group)or continuous seamless community hospital integrated management mode(the observation group),the intervention effect and quality of life were recorded.Results The total effective rate in the observation group was significantly higher than that in the control group(93.75%vs 75.00%,P<0.05).Hamilton Rating Scale of Anxiety(HAMA),Hamilton Rating Scale of Depression(HAMD)and Pittsburgh Sleep Quality Index(PSQI)scores in the observation group were significantly lower than those in the control group after intervention(P<0.01).There was no significant difference in cardiac function between the two groups before intervention(P>0.05).After intervention,the levels of left ventricular ejection fraction(LVEF)and left ventricular end-diastolic diameter(LVEDD),left atrial diameter(LAD),ΔPm in the observation group was significantly better than those in the control group(P<0.05).There was no significant difference in quality of life score between the two groups before intervention(P>0.05),but it was significantly improved after intervention(P<0.05),and the score in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Continuous seamless community hospital integrated management mode is beneficial to improve the intervention effect of elderly patients with CHF,which can improve the psychological and sleep state,and quality of life of patients,it is worthy of clinical promotion.
