摘  要:目的:探讨氧化应激指标与小儿复杂型热性惊厥的关系及对发作后脑损伤的预测价值研究。方法:选择52例复杂型热性惊厥患儿作为复杂型组,另选同期的52例单纯型热性惊厥患儿作为单纯型组。比较两组血清氧化应激指标[丙二醛(MDA)、8-羟基脱氧鸟苷酸(8-OHdG)和谷胱甘肽过氧化物酶(GSH-Px)],使用Pearson相关性分析氧化应激指标与复杂型热性惊厥患儿抽搐持续时间、抽搐次数的关系,通过受试者工作特征(ROC)曲线下面积(AUC)评价氧化应激指标对发作后脑损伤的预测效能。结果:复杂型组血清MDA、8-OHdG水平明显高于单纯型组,GSH-Px水平明显低于单纯型组(P<0.05);经Pearson相关性分析,复杂型热性惊厥患儿血清MDA、8-OHdG水平均与抽搐持续时间、抽搐次数呈正相关(均P<0.05),GSH-Px水平与抽搐持续时间、抽搐次数呈负相关(P<0.05);脑损伤组血清MDA、8-OHdG水平明显高于非脑损伤组,GSH-Px水平明显低于非脑损伤组(均P<0.05);经多因素Logistic回归分析,MDA、8-OhdG、GSH-Px均是复杂型热性惊厥患儿发作后脑损伤的独立预测因素(均P<0.05);经ROC曲线分析,血清MDA、8-OHdG联合GSH-Px预测复杂型热性惊厥发作后脑损伤的AUC为0.924。结论:小儿复杂型热性惊厥可能与血清MDA、8-OhdG水平升高和GSH-Px水平降低有关,三者联合预测发作后脑损伤的效能较好,值得临床进一步研究应用。Objective:To investigate the relationship between oxidative stress indexes and complex febrile convulsion in children and its predictive value for post-attack brain injury.Methods:A total of 52 children with complex febrile convulsion were selected as the complex type group,and 52 children with simple febrile convulsion during the same period were selected as the simple type group.Serum oxidative stress indexes[Malondialdehyde(MDA),8-hydroxy-deoxyguanylate(8-OHDG)and glutathione peroxide(GSH-PX)]were compared between the two groups.Pearson correlation analysis was used to analyze the relationship between oxidative stress indexes and duration and times of convulsions in children with complex febrile convulsion.The AUC of ROC curve was used to evaluate the predictive efficacy of oxidative stress indicators for post-attack brain injury.Results:The levels of serum MDA and 8-OHDG in the complex type group were significantly higher than those in the simple type group,and the level of GSH-Px was significantly lower than that in the simple type group(all P<0.05).The levels of MDA and 8-OHDG were positively correlated with duration of convulsions and times of convulsions,while GSH-Px was negatively correlated with duration of convulsions and times of convulsions(all P<0.05).The levels of MDA and 8-OHDG in the brain injury group were significantly higher than those in the non-brain injury group,and the level of GSH-Px was significantly lower than that in the non-brain injury group(all P<0.05).Multivariate Logistic regression analysis showed that MDA、8-OHDG and GSH-Px were independent risk factors for brain injury in children with complex febrile convulsion(all P<0.05).ROC curve analysis showed that the AUC of MDA,8-OHDG combined with GSH-Px in predicting brain injury after complex febrile convulsions was 0.924.Conclusion:The complex febrile convulsion may be associated with increased levels of MDA,8-OHDG and decreased level of GSH-Px,and their combined efficacy in predicting post-attack brain injury is good,which is wort
