摘  要:目的:肺动脉造影结合间接下肢静脉造影(CTVPA)与双功能超声检查比较,评价CT和超声(DUS)对深静脉血栓(DVT)检测的一致性及差异性。方法:回顾性分析2020年10月-2021年9月经DSA确诊的88例DVT病例,分析其双功能超声和CTVPA肺动脉及腹部静脉、盆腔静脉及腿部静脉形态学表现,CT应用128排MSCT一次注射碘海醇后进行肺动脉及间接腹腔静脉、盆腔静脉和腿部静脉联合扫描,范围从肺尖至肺底、下腔静脉心脏入口处至踝关节(肺尖到脚踝)。由两位影像科医生观察图像,评价依据每个静脉段记录栓子的位置和数量。结论:CTVPA与双功能超声比较,CT对深静脉血栓检出率明显高于超声,而双功能超声有无创、操作简便,可通过直接及间接征象等多指标进行重复检查等优点。Objective:To compare pulmonary arteriography combined with indirect lower extremity venography(CTVPA)with dual function ultrasound(DUS),and to evaluate the consistency and difference of CT and DUS in detecting deep vein thrombosis(DVT).Methods:88 cases of DVT diagnosed by DSA from October 2020 to September 2021 were analyzed retrospectively.The morphological features of the pulmonary artery,abdominal vein,pelvic vein and leg vein of CTVPA were analyzed by duplex ultrasound and CTVPA.The pulmonary artery,indirect abdominal cavity vein,pelvic vein and leg vein were scanned by CT after a single injection of iohexol with 128 row MSCT,ranging from the top of the lung to the bottom of the lung,and from the heart inlet of the inferior vena cava to the ankle joint(from the top of the lung to the ankle).Two radiologists observed the images and evaluated the location and number of emboli recorded according to each vein segment.Conclusion:compared with the dual function ultrasound,the detection rate of CT in pulmonary embolism is significantly higher than that of ultrasound,while the dual function ultrasound has the advantages of non-invasive,simple operation,and can be repeated through multiple indicators such as direct and indirect signs.
