摘 要:目的:基于辩证行为疗法,结合行为改变阶段理论设计适用于降低大学生非自杀性自伤行为的引导自助程式干预方案,评估其即时和短期效果,为促进高校学生心理健康提供参考。方法:选取青少年非自杀性自伤评定问卷(ANSAQ)评分≥1分的大一新生144人随机分为干预组(n=121人)和等待组(n=123人),干预组接受8周引导自助程式干预,分别于干预前、干预8周结束时和干预后12周时采用ANSAQ问卷、CES-D量表、PSQI指数施测评估干预效果。结果:重复测量方差分析显示:各变量组间效应(F=13.427,18.571,8.516;P<0.01)、时间效应(F=9.613,13.296,5.993;P<0.05)、分组×时间(F=21.382,38.437,9.893;P<0.01)交互效应显著。简单效应分析显示:干预组ANSAQ、CES-D评分及PSQI指数显著下降,均有统计学差异(F=6.860,12.979,6.715;P<0.01);而等待组ANSAQ、CES-D评分及PSQI指数无显著变化,均无统计学差异。干预组干预8周结束时ANSAQ、CES-D评分及PSQI指数低于基线(t=-5.140,-8.267,-3.543;P<0.05),干预结束后12周ANSAQ、CES-D评分及PSQI指数低于干预8周结束时(t=-6.209,-7.381,-2.517;P<0.05),组内比较均有统计学差异;等待组各时间点两两之间差异均无统计学差异。结论:基于辩证行为疗法的引导自助程式干预能有效缓解大学新生抑郁症状,降低非自杀性自伤行为水平,改善睡眠质量。Objective:To develop a guided self-help program intervention based dialectical behavior therapy and transtheoretical model for college students with non-suicidal self-injury and to evaluate its effect after intervention and 12-weeks follow-up,so to provide a reference for promoting college studentsmental health.Methods:Totally 144 freshmen with ANSAQ score≥1 were randomly assigned to the intervention group(n=121)and the waiting group(n=123).The intervention group received Guided Self-Help Program Intervention for 8 weeks.The ANSAQ,CES-D and PSQI were measured at baseline,8 weeks and 12weeks after intervention to evaluate the effect of intervention.Results:Repeated measurement analysis of variance showed that the group effect(F=13.427,18.571,8.516;P<0.01),time effect(F=9.613,13.296,5.993;P<0.05)and group×time interaction(F=21.382,38.437,9.893;P<0.01)were statistically significant.Simpleeffect analysis showed that ANSAQ,CES-Dscores and PSQI scores had significant declines in theintervention group(F=6.860,12.979,6.715;P<0.01),when in the waiting group there were no significant decline(P>0.05).In intervention group,the ANSAQ,CES-Dscores and PSQI scores were lower at the end of 8 weeks intervention than at baseline(t=-5.140,-8.267,-3.543;P<0.05),and they were lower at 12 weeks after intervention than at the end of 8 weeks intervention(t=-6.209,-7.381,-2.517;P<0.05).There were no significant differences between the intervention group and the waiting groupat each time point(P>0.05).Conclusion:A guided self-help program intervention based DBT can effectively relieve the depressive symptoms,reduce the level of non-suicidal self-injury and improve the sleep quality of freshmen.
