促性腺激素释放激素激动剂辅助腹腔镜下卵巢囊肿剥离术对患者外周血TNF-α、IL-6、NLRs表达水平的影响

摘　　要：目的 探讨腹腔镜下卵巢囊肿剥离术后使用促性腺激素释放激素激动剂(GnRH-a)辅助治疗对患者外周血肿瘤坏死因子-α(TNF-α)与白细胞介素-6(IL-6)及核苷酸结合寡聚域样受体(NLRs)表达水平的影响。方法选取2020年1月至2021年10月哈尔滨市第一医院收治的84例卵巢囊肿患者为研究对象,按照随机数字表法将患者分成观察组(42例)与对照组(42例),对照组应用腹腔镜下卵巢囊肿剥离术治疗,观察组在对照组的基础上,术后应用Gn RH-a辅助治疗,观察两组术后6个月的疗效与妊娠率以及术前术后血清TNF-α与IL-6表达水平、NLRs家族NOD1与NOD2 m RNA表达水平。结果 观察组总有效率与妊娠率分别为95.24%、78.57%,对照组分别为78.57%、50.00%,观察组总有效率与妊娠率均高于对照组,差异有统计学意义(P<0.05);治疗后,两组患者的TNF-α、IL-6、NOD1 m RNA、NOD2 m RNA水平均低于本组治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论 Gn RH-a辅助腹腔镜下卵巢囊肿剥离术治疗,可以取得良好的治疗效果,提高临床妊娠率,降低外周血TNF-α、IL-6及NLRs表达水平。Objective To investigate the effect of gonadotropin-releasing hormone agonist(GnRH-a)adjuvant therapy on the expression levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and nucleotide binding oligomerization domain-like receptors(NLRs)in peripheral blood of patients after laparoscopic ovarian cystectomy.Methods A total of 84 patients with ovarian cysts admitted to Harbin First Hospital from January 2020 to October 2021 were selected as the research objects.According to the random number table method,the patients were divided into observation group(42 cases)and control group(42 cases).The control group was treated with laparoscopic ovarian cyst dissection,and the observation group was treated with GnRH-a adjuvant therapy on the basis of the control group.The efficacy and pregnancy rate of the two groups were observed after 6 months of GnRH-a adjuvant therapy,and the serum levels of TNF-αand IL-6,NOD1 and NOD2 mRNA expression levels of NLRs family before and after surgery were observed.Results The total effective rate and pregnancy rate of the observation group were 95.24%and 78.57%,respectively,which of the control group were 78.57%and 50.00%,respectively.The total effective rate and pregnancy rate of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of TNF-α,IL-6,NOD1 mRNA and NOD2 mRNA in the two groups were lower than those before treatment,and indexes in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion GnRH-a assisted laparoscopic ovarian cyst dissection can achieve good therapeutic effect,improve the clinical pregnancy rate,and reduce the expression levels of TNF-α,IL-6 and NLRs in peripheral blood.
