摘 要:目的:探讨艾滋病合并真菌感染患者自然杀伤(NK)细胞亚群与炎性因子水平的相关性。方法:回顾性分析2020年1月—2021年6月湖北省英山县人民医院收治的艾滋病患者的临床资料,依据其是否合并真菌感染将其分为非真菌感染组(40例)及真菌感染组(70例),并选取同期30例体检健康者的体检资料作为对照组。所有研究对象进行NK细胞亚群及炎性因子水平检测,比较三组NK细胞亚群分布及炎性因子表达情况,分析NK细胞亚群与炎性因子水平的相关性。结果:非真菌感染组及真菌感染组外周血CD56 bright NK、CD56 dim NK、CD56-CD16+NK、NK水平低于对照组,真菌感染组CD56 bright NK、CD56 dim NK、CD56-CD16+NK、NK水平低于非真菌感染组,差异有统计学意义(P<0.05)。非真菌感染组及真菌感染组血清肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)、白细胞介素-8(IL-8)、白细胞介素-6(IL-6)水平高于对照组,真菌感染组血清TNF-α、IL-10、IL-8、IL-6水平高于非真菌感染组,差异有统计学意义(P<0.05)。血清TNF-α、IL-10、IL-6水平与外周血CD56 bright NK、CD56 dim NK、NK水平呈负相关关系,血清IL-8与外周血CD56-CD16+NK水平呈正相关关系(P<0.05)。结论:在艾滋病合并真菌感染患者中,随着病情进展外周血NK细胞亚群水平降低,炎性因子水平升高,且NK细胞亚群与血清炎性因子水平存在显著的相关性。Objective:To investigate the correlation between natural killer(NK)cell subsets and inflammatory factor levels in AIDS patients with fungal infection.Methods:The clinical data of AIDS patients admitted to Yingshan County People's Hospital in Hubei Province from January 2020 to June 2021 were retrospectively analyzed.According to whether they were complicated with fungal infection,they were divided into non-fungal infection group(40 cases)and fungal infection group(70 cases).The physical examination data of 30 healthy people in the same period were selected as the control group.NK cell subset and inflammatory factor levels in all subjects were detected.The distribution of NK cell subsets and the expression of inflammatory factors were compared,and the correlation between NK cell subsets and the levels of inflammatory factors was analyzed.Results:The levels of CD56 bright NK,CD56 dim NK,CD56-CD16+NK and NK in the non-fungal infection group and the fungal infection group were lower than those in the control group,and the CD56 bright NK,CD56 dim NK,CD56-CD16+NK and NK levels in the fungal infection group were lower than those in the non-fungal infection group,and the differences were statistically significant(P<0.05).The levels of serum tumor necrosis factor-α(TNF-α),interleukin-10(IL-10),interleukin-8(IL-8)and interleukin-6(IL-6)in the non-fungal infection group and the fungal infection group were higher than those in the control group,and the serum levels of TNF-α,IL-10,IL-8 and IL-6 in the fungal infection group were higher than those in the non-fungal infection group,and the differences were statistically significant(P<0.05).Serum TNF-α,IL-10 and IL-6 levels were negatively correlated with peripheral blood CD56 bright NK,CD56 dim NK and NK levels,and serum IL-8 was positively correlated with peripheral blood CD56-CD16+NK levels(P<0.05).Conclusion:In AIDS patients with fungal infection,the levels of NK cell subsets in peripheral blood decrease and the levels of inflammatory factors increase with the progress o
