中药抗感染洗剂治疗手外伤术后的疗效观察

摘　　要：目的:探讨手外伤术后使用具有抗感染功能的中药洗剂(医院自制)促进伤口愈合的临床效果,以评价其治疗作用。方法:将陕西中医药大学附属医院骨病科进行治疗的60例手外伤术后患者作为研究主体进行分析,随机划分成对照组和观察组,每组30例。对照组给予康复训练治疗,内容包括肌腱恢复训练等疗法,连续治疗1个月。观察组在对照组治疗方案基础上给予中药抗感染洗剂治疗,将药方加入到1500 mL水中煮沸,煎至500 mL左右取汁,先用洗剂熏蒸患者患处,待药水温度下降,对患处进行熏洗浸泡约30 min。每日2次,连续治疗1个月。对比治疗前后两组患者的VAS评分、上肢功能指数(UEFI)、临床疗效、血液循环评分、手指总活动度(TAM)的变化情况。结果:治疗前两组指标具有可比性,在VAS评分、上肢功能指数(UEFI)、临床疗效、血液循环评分、手指总活动度(TAM)的指标比较,差异无统计学意义(P>0.05)。两组患者在治疗后的VAS评分、上肢功能指数、血液循环评分和临床疗效均有所改善,其中,观察组患者的VAS评分明显低于对照组(P<0.05);观察组患者的血液循环评分、TAM、临床疗效、UEFI指标明显高于对照组(P<0.05)。结论:中药抗感染洗剂对手外伤术后的患者有一定的临床疗效,可以促进手外伤患者术后的伤口愈合,有效抗感染,并且可以有效降低患者的VAS评分,提高血液循环评分,TAM和UEFI指标,提高临床治疗效果,改善患者的疼痛不适症状,增强患者的治疗信心,加速手部恢复,提升患者的生活质量,是一种具有可行性与科学性的治疗方式,值得在临床推广应用。Objective:To explore the clinical efficacy of self-made Chinese medicinal anti-infection lotion in promoting wound healing after hand injury.Methods:60 patients with postoperative hand injury were randomly divided into the control group and the observation group,with 30 cases in each group.The control group was treated with rehabilitation training for 1 month;on which basis,the observation group was also treated with Chinese medicinal anti-infection lotion.Chinese medicinal was made into the decoction of 500 mL.The infected area was fumigated with the lotion,when the lotion temperature was lower,the affected area was fumigated and soaked for 30 minutes,twice a day,once a month.The scores of VAS,UEFI,blood circulation and TAM were compared between before and after the treatment in the two groups.Clinical efficacy was also compared between the two groups.Results:There were no statistical differences in the scores of VAS,UEFI,blood circulation and TAM between the two groups before the treatment(P>0.05).The scores of VAS,UEFI and blood circulation and TAM were all significantly improved after the treatment compared with those before the treatment in the two groups(P<0.05).The score of VAS was remarkably lower in the observation group than that in the control group(P<0.05);the scores of blood circulation,TAM,clinical efficacy and UEFI were significantly higher in the observation than those in the control group(P<0.05).Conclusion:Chinese medicinal anti-infection lotion has a significant effect in treatment of postoperative hand injury.It can promote wound healing,effectively resists infection,significantly reduce VAS score,increase the scores of blood circulation,TAM and UEFI,improve clinical efficacy and discomforts,and enhance confidence of the patients.It is a feasible and scientific treatment method that can accelerate hand rehabilitation and improve patients’life quality.
