无导针器技术联合生理盐水对超声引导下改良塞丁格外周置入中心静脉导管置管术患者一次性穿刺成功率的影响

摘　　要：目的探究无导针器技术联合生理盐水对超声引导下改良塞丁格(Seldinger)外周置入中心静脉导管(PICC)置管术患者一次性穿刺成功率的影响。方法选取2019年2月至2020年2月抚州市第一人民医院收治的70例行超声引导改良Seldinger PICC置管术的患者作为研究对象,采用随机数字表法将患者分为对照组和试验组,各35例。对照组采用超声引导套件进行穿刺,试验组采用无导针器技术联合生理盐水进行穿刺,比较两组患者的一次性穿刺成功率、疼痛评分以及并发症发生情况。结果试验组一次穿刺成功率、一次置管成功率以及置管总成功率高于对照组,差异有统计学意义(P<0.05);试验组的疼痛视觉模拟量表(VAS)评分低于对照组,差异有统计学意义(P<0.05);试验组穿刺期间的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论对超声引导下改良Seldinger PICC置管术患者采用无导针器技术联合生理盐水穿刺可显著提升一次性穿刺成功率及置管成功率,缓解患者疼痛并减少并发症发生情况。Objective To investigate the effect of acuductor-free technique combined with normal saline on the success rate of once-time puncture in patients undergoing peripherally inserted central catheter(PICC)catheterization with ultrasound-guided modified Seldinger technique.Methods Seventy patients undergoing ultrasound-guided modified Seldinger PICC catheterization admitted to the First People's Hospital of Fuzhou from February 2019 to February 2020 were selected as the research objects.The patients were divided into control group and experimental group by random number table method,with 35 cases in each group.The control group was puncture by ultrasonic guide kit,and the experimental group was puncture by needle guide less technology combined with normal saline.One-time puncture success rate,pain score and complications were compared between the two groups.Results The success rate of one puncture,one catheterization and the total success rate of catheterization in the experimental group were higher than those in the control group,and the differences were statistically significant(P<0.05).Visual analogue scale(VAS)score of experimental group was lower than that of control group,and the difference was statistically significant(P<0.05).The incidence of complications during puncture in experimental group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion Applying acuductor-free technique combined with normal saline to patients undergoing PICC catheterization with ultrasound-guided modified Seldinger technique can significantly increase the success rates of once-time puncture and catheterization,alleviate pain and reduce complications.
