摘 要:目的探讨双水平气道正压通气(BiPAP)与压力调节容量控制通气(PRVC)治疗慢性阻塞性肺疾病急性加重期(AECOPD)的效果及对血气、呼吸力学指标的影响。方法选取2020年4月至2021年10月我院收入AECOPD患者80例,按照随机数字表法分为观察组与对照组各40例,观察组提供BiPAP治疗,对照组提供PRVC治疗,对比两组患者临床效果差异及治疗前后血气指标[pH值、氧饱和度(SaO_(2))、氧分压(PaO_(2))]、呼吸力学指标[气道阻力(Raw)、平均气道压(Paw)、动态顺应性(Cdyn)]变化。结果治疗后,观察组临床疗效高于对照组,差异有统计学意义(P<0.05);两组患者pH值、SaO_(2)、PaO_(2)及Cdyn均高于同组治疗前,且观察组高于对照组;Raw、Paw均低于同组治疗前,且观察组低于对照组。结论BiPAP治疗可有效降低AECOPD患者气道压力,提高肺部顺应性,增强氧合,有利于缓解患者临床症状并改善预后。Objective:To investigate the effect of bilevel positive airway pressure(BiPAP)and pressure-regulated volume-controlled ventilation(PRVC)in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and their effects on blood gases and respiratory mechanics.Methods A total of 80 patients with AECOPD admitted to our hospital from April 2020 to October 2021 were selected and divided into an observation group and a control group with 40 cases in each group according to the random number table method.The observation group was provided with BiPAP treatment,and the control group was provided with PRVC treatment.Differences in clinical effect of patients and blood gas indexes before and after treatment[pH value,oxygen saturation(SaO_(2)),partial pressure of oxygen(PaO_(2))],respiratory mechanics indexes[airway resistance(Raw),mean airway pressure(Paw),dynamic compliance(Cdyn)]changes.Results After treatment,the clinical efficacy of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05);Raw and Paw were lower than those in the same group before treatment,and the observation group was lower than the control group.Conclusion BiPAP therapy can effectively reduce airway pressure,improve lung compliance,and enhance oxygenation in patients with AECOPD,which is beneficial to relieve clinical symptoms and improve prognosis.
