肝动脉化疗栓塞联合射频消融技术治疗大肝癌的疗效分析

摘　　要：目的研究在大肝癌治疗中采用肝动脉化疗栓塞(transarterial chemoembolisation,TACE)联合射频消融技术(radiofrequency ablation,RFA)的疗效。方法选取2018年4月—2021年4月齐齐哈尔医学院附属第二医院介入科收治的40例大肝癌患者,按照奇数偶数分组法分为观察组与对照组,各20例。对照组采取TACE治疗,观察组采用TACE+RFA治疗。统计两组疾病缓解率,分析两组肝功能指标及随访1年后的生存率。结果观察组疾病缓解率为90.00%,高于对照组的60.00%,差异有统计学意义(χ2=4.800,P<0.05);治疗6个月后,两组的ALT、TBIL水平比较,差异无统计学意义(P>0.05),而观察组MTD、AFP水平低于对照组,差异有统计学意义(P<0.05);随访1年,观察组的生存率为95.00%高于对照组的70.00%,差异有统计学意义(P<0.05)。结论TACE联合RFA在治疗大肝癌中,疗效好,可改善肝功能指标。Objective To investigate the efficacy of hepatic arterial chemoembolization(TACE)combined with radiofrequency ablation(RFA)in the treatment of large liver cancer.Methods Forty patients with large liver cancer admitted to the Department of Intervention of the Second Affiliated Hospital of Qiqihar Medical College from April 2018 to April 2021 were selected and divided into observation group and control group by odd-even grouping method,with 20 cases in each group.The control group received TACE treatment,and the observation group received TACE+RFA treatment.The remission rate,liver function index and survival rate of the two groups after 1 year follow-up were analyzed.Results The disease remission rate in the observation group was 90.00%higher than that in the control group(60.00%),and the difference was statistically significant(χ^(2)=4.800,P<0.05).After 6 months of treatment,there was no statistically significant difference in the levels of ALT and TBIL between the two groups(P>0.05),while the level of MTD and AFP in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).Following up for one year,the survival rate of the observation group was 95.00%higher than that of the control group,70.00%,the difference was statistically significant(P<0.05).Conclusion TACE combined with RFA is effective in the treatment of large liver cancer and can improve liver function indexes.
