骨搬运联合膜诱导术治疗胫骨创伤后骨髓炎40例

摘　　要：目的:分析骨搬运联合膜诱导术治疗胫骨创伤后骨髓炎的疗效。方法:回顾性分析2016年5月至2020年5月采用骨搬运联合膜诱导术治疗的40例胫骨创伤后骨髓炎患者的临床资料,分析所有患者的临床疗效、临床指标、简明健康调查问卷(SF-36)评分及并发症发生情况。结果:术后治疗总有效率为97.50%(39/40),其中愈合28例,基本愈合11例,无效1例;总带架时间为(10.48±2.62)个月,总治疗时间为(12.19±6.76)个月;术后3,6,9,12个月SF-36评分明显高于术前[(71.35±6.93)分、(81.90±6.27)分、(84.89±6.25)分、(86.48±6.06)分vs(39.14±6.38)分](t=21.627,30.233,32.397,34.026,P<0.001);术后并发症发生率为7.50%(3/40),其中针道感染2例,骨髓炎复发1例。结论:胫骨创伤后骨髓炎患者予以骨搬运联合膜诱导术治疗,在减少并发症、提高生活质量及促进早日康复方面具有良好的效果。Objective:To analyze the efficacy of the therapy with bone transport combined with membrane induction in osteomyelitis after tibial trauma.Methods:The clinical datas of 40 patients with osteomyelitis after tibial trauma treated by bone transport combined with membrane induction from May 2016 to May 2020 were retrospectively analyzed.The clinical efficacy,clinical index,concise health questionnaire(SF-36)score and complication of all patients were analyzed after surgery.Results:The therapy total effective rate was 97.50%(39/40),including healing was 28 cases,basic healing was 11 cases and invalid was 1 case.The total band frame time was(10.48±2.62)months and the total treatment time was(12.19±6.76)months.The SF-36 score at 3,6,9 and 12 months after surgery were evidently higher than those before surgery[(71.35±6.93),(81.90±6.27),(84.89±6.25),(86.48±6.06)vs(39.14±6.38)](t=21.627,30.233,32.397,34.026,P<0.001).The incidence of postoperative complications was 7.50%(3/40),including needle infection was 2 cases,recurrence of osteomyelitis was 1 case.Conclusion:The therapy of bone transport combined with induction membrane in onsteomyelitis after tibial trauma has a good efficacy in reducing complications,improving quality of life and promoting early recovery.
