心电图平板运动试验与动态心电图在冠心病诊断中的效能比较

摘　　要：目的:比较心电图平板运动试验与动态心电图在冠心病诊断中的效能。方法:选取2019年11月至2022年6月该院98例疑似冠心病患者进行前瞻性研究,均行心电图平板运动试验和动态心电图检查。以冠状动脉造影结果为“金标准”,统计并比较心电图平板运动试验和动态心电图对冠心病的检出结果、诊断效能及冠心病病变检出情况。结果:冠状动脉造影结果显示,98例疑似冠心病患者中,阳性70例,阴性28例;心电图平板运动试验检出阳性68例,阴性30例;动态心电图检出阳性65例,阴性33例;心电图平板运动试验与动态心电图诊断冠心病的特异度、误诊率、阳性预测值比较,差异均无统计学意义(P>0.05);心电图平板运动试验诊断冠心病的灵敏度、准确度、阴性预测值均高于动态心电图,漏诊率低于动态心电图,差异有统计学意义(P<0.05);心电图平板运动试验对冠心病病变的检出率为97.14%(68/70),动态心电图对冠心病病变的检出率为92.86%(65/70),两种检查方法对冠心病病变的检出率比较,差异无统计学意义(P>0.05)。结论:心电图平板运动试验诊断冠心病的效能高于动态心电图。Objective:To compare efficacies of electrocardiogram treadmill exercise test and dynamic electrocardiogram in diagnosis of coronary heart disease.Methods:A prospective study was conducted on 98 patients with suspected coronary heart disease in the hospital from November 2019 to June 2022.All patients underwent electrocardiogram treadmill exercise test and dynamic electrocardiogram examination.Using the results of coronary angiography as the“gold standard”,the detection results,the diagnostic efficacies and the detection of coronary heart disease lesions of electrocardiogram treadmill exercise test and dynamic electrocardiogram in the diagnosis of coronary heart disease were statistically analyzed and compared.Results:The coronary angiography showed that among the 98 patients with suspected coronary heart disease,70 were positive and 28 were negative;the electrocardiogram treadmill exercise test showed 68 were positive and 30 were negative;and dynamic electrocardiogram examination showed there were 65 positive cases and 33 negative cases.There were no significant differences in the specificity,misdiagnosis rate and positive predictive value between electrocardiogram treadmill exercise test and dynamic electrocardiogram in the diagnosis of coronary heart disease(P>0.05).The sensitivity,accuracy and negative predictive value of electrocardiogram treadmill exercise test in the diagnosis of coronary heart disease were higher than those of dynamic electrocardiogram,the missed diagnosis rate was lower than that of dynamic electrocardiogram,and the differences were statistically significant(P<0.05).The detection rate of electrocardiogram treadmill exercise test for coronary heart disease was 97.14%(68/70),The detection rate of dynamic electrocardiogram for coronary heart disease was 92.86%(65/70),There were no significant differences in the specificity,the detection rate of two examination methods for coronary heart disease(P>0.05).Conclusions:The efficacy of electrocardiogram treadmill exercise test in the diagnosis
