Foley尿管球囊与缩宫素在足月妊娠引产中的应用效果比较

摘　　要：目的:比较Foley尿管球囊与缩宫素在足月妊娠引产中的应用效果。方法:回顾性分析2020年1月至2021年6月该院收治的80例足月妊娠引产产妇的临床资料,按照引产方式不同将其分为观察组和对照组各40例。对照组采用缩宫素引产,观察组采用Foley尿管球囊引产,比较两组引产前后宫颈Bishop评分、临床指标(引产至临产时间、产后出血量、总产程)、母婴结局(阴道分娩率和新生儿窒息、胎儿宫内窘迫发生率)及并发症发生率。结果:观察组引产后宫颈Bishop评分、阴道分娩率均高于对照组,差异有统计学意义(P<0.05);观察组引产至临产时间短于对照组,产后出血量少于对照组,胎儿宫内窘迫发生率低于对照组,差异有统计学意义(P<0.05);两组总产程、并发症发生率、新生儿窒息发生率比较,差异均无统计学意义(P>0.05)。结论:Foley尿管球囊应用于足月妊娠引产,可提高产妇宫颈成熟度,缩短引产至临产时间,减少产后出血量,优化母婴结局,效果优于缩宫素引产。Objective:To compare effects of Foley catheter balloon and Oxytocin in induced labor of full-term pregnancy.Methods:The clinical data of 80 cases of induced labor of full-term pregnancy in the hospital from January 2020 to June 2021 were retrospectively analyzed.They were divided into observation group and control group according to different methods of induced labor,40 cases in each.The control group was induced by Oxytocin,while the observation group was induced by Foley catheter balloon.The cervical Bishop score,the clinical indicator levels(time from induced labor to labor,postpartum hemorrhage,total labor process),the maternal and infant outcomes(vaginal delivery rate,incidence of neonatal asphyxia,incidence of fetal distress),and the incidence of complications were compared between the two groups before and after induced labor.Results:The cervical Bishop score and the vaginal delivery rate in the observation group were significantly higher than those in the control group after induced labor,and the differences were statistically significant(P<0.05).The time from induced labor to labor in the observation group was shorter than that in the control group,the amount of postpartum hemorrhage was less than that in the control group,the incidence of fetal distress was lower than that in the control group,and the differences were statistically significant(P<0.05).However,there were no statistically significant differences between the two groups in the total labor process,the incidence of neonatal asphyxia and the incidence of complications(P>0.05).Conclusions:Foley catheter balloon can improve the cervical maturity,shorten the time from induced labor to labor,reduce the postpartum hemorrhage,and optimize the maternal and infant outcomes.Moreover,it is superior to Oxytocin induced labor.
