摘 要:目的:分析儿童肝脏真菌感染的CT影像学特征对该病的诊断价值。方法:分析11例经临床证实的儿童肝脏真菌性感染患儿的临床资料及CT影像表现。结果:11例病例中,其中单发病灶1例,多发病灶10例。所有病灶在CT平扫图像上均表现为低密度或等密度,病例CT三期增强扫描上表现为三种模式,Ⅰ型:CT三期增强扫描均显示“靶环征”,为不均匀环状强化。Ⅱ型:病变在动脉期和门静脉期呈等密度或低密度,延迟期呈等密度。Ⅲ型:病变在动脉期和门静脉期呈等密度或低密度,延迟期呈环状强化。病灶多不影响肝脏血管,增强扫描可显示正常肝脏血管走行于病灶边缘或内部,且大多数病灶外侧的肝实质在动脉期表现为一过性异常高灌注。所有患者经抗真菌治疗后,病变完全消失或缩小。结论:分析肝脏增强肝脏CT多期(平扫、动脉期、门静脉期、延迟期)扫描影像改变以及临床资料,有助于该病早期诊断、早期治疗。Objective To investigate and analyze the CT imaging features of fungal infection of the liver in children,so as to improve the diagnostic value of the disease.Methods The clinical data and CT findings of 11 children with clinically proven fungal infection of the liver were analyzed.Results Of the 11 cases,1 had single lesion and 10 had multiple lesions.All lesions showed low density or isodensity on CT plain scan images.In this group,three patterns were shown on the third phase enhanced CT scan.TypeⅠ:“target ring sign”was shown on the third phase enhanced CT scan,which was non-uniform annular enhancement.TypeⅡ:The lesion was of equal or low-density in arterial phase and portal vein phase,and of equal density in delayed phase.TypeⅢ:The lesions were isodensity or low-density in arterial and portal venous phases,and annular enhancement in delayed phase.most of the lesions did not affect the hepatic vessels.Enhanced scanning could show that the normal hepatic vessels ran along the edge or inside of the lesions,and most of the hepatic parenchyma outside the lesions showed transient abnormal high perfusion in the arterial phase.After antifungal treatment,the lesion completely disappeared or shrunk in all patients.Conclusion Analysis of multiphase(plain scan,arterial phase,portal vein phase,delayed phase)CT image changes and clinical data of liver enhancement is helpful for early diagnosis,early treatment and detection of the disease.
