消瘿清热汤对甲亢伴胫前黏液性水肿患者的效果及药理分析

摘　　要：目的分析消瘿清热汤治疗甲状腺功能亢进症(甲亢)伴胫前黏液性水肿患者的临床效果及药理作用。方法60例甲亢伴胫前黏液性水肿患者,通过信封法随机分为对照组和试验组,各30例。对照组应用甲巯咪唑联合穴位注射治疗,试验组在对照组基础上联合消瘿清热汤治疗。观察比较两组患者治疗前后中医证候积分、甲状腺激素水平及皮损面积。结果治疗后,试验组中医证候积分(7.41±1.41)分、三碘甲状腺原氨酸(T3)(5.33±1.24)pmol/L、四碘甲状腺原氨酸(T4)(1.73±0.32)pmol/L均低于对照组的(10.32±1.57)分、(7.51±1.41)pmol/L、(1.95±0.44)pmol/L,皮损面积(30.42±7.34)cm^(2)小于对照组的(42.32±9.71)cm^(2),差异有统计学意义(P<0.05)。结论消瘿清热汤治疗甲亢伴胫前黏液性水肿患者的临床效果显著,值得临床推广。Objective To analyze the clinical effect of Xiaoying Qingre decoction in the treatment of hyperthyroidism with pretibial myxedema and its pharmacological role.Methods A total of 60 patients with hyperthyroidism and pretibial myxedema were randomly divided into control group and experimental group by envelope method,with 30 cases in each group.The control group was treated with methimazole and acupoint injection,and the experimental group was treated with Xiaoying Qingre decoction on the basis of the control group.The traditional Chinese medicine syndrome score,thyroid hormone level and skin lesion area were observed and compared before and after treatment between the two groups.Results After treatment,the experimental group had traditional Chinese medicine syndrome score of(7.41±1.41)points,triiodothyronine(T3)of(5.33±1.24)pmol/L,and tetraiodothyronine(T4)of(1.73±0.32)pmol/L,which were lower than(10.32±1.57)min,(7.51±1.41)pmol/L,and(1.95±0.44)pmol/L in the control group;the skin lesion area of(30.42±7.34)cm^(2) in the experimental group was less than that of(42.32±9.71)cm^(2) in the control group;the differences were statistically significant(P<0.05).Conclusion Xiaoying Qingre decoction has significant clinical effect for the treatment of hyperthyroidism with pretibial myxedema,and is worthy of clinical promotion.
