基于数据挖掘探讨温成平教授治疗类风湿关节炎的用药规律

摘　　要：目的 基于数据挖掘分析温成平教授治疗类风湿关节炎患者的用药规律.方法 通过浙江中医药大学联合中国信息通信研究院研发的类风湿性关节炎(rheumatoid arthritis,RA)辅助诊疗系统,将临床脉案录入数据库,采用Apriori关联规则算法、K-means聚类等数据分析方法,分析温成平教授164则治疗类风湿关节炎处方中的药物、症状频次,药物之间及药物和症状之间的关联规则以及症状的聚类分析等,提炼其对此病的治疗经验.结果 根据数据分析,处方中使用频次最高的药物有金银花、蜈蚣、甘草,中药药对中支持度最高的为蜈蚣、雷公藤与菟丝子药对,提升度最高的为防己和薏苡仁对透骨草药对,置信度最高的为垂盆草、山药和菟丝子组.结论 根据RA患者脾虚与湿毒瘀互结的病机特点,温成平教授在治疗RA上以解毒通络为主,并注重健脾祛湿,顾护脾胃,此法对于类风湿性关节炎的治疗有较好的临床意义.Objective To analyze the medication rule of Professor Wen Chengping in the treatment of patients with active rheumatoid arthritis based on the Apriori association rule.Method By the rheumatoid arthritis(rheumatoid arthritis,RA)aided diagnosis system developed by Zhejiang Chinese medical university and China academy of information communication,the clinical MaiAn was entered into the database.By using the Apriori algorithm of association rules,K means clustering data analysis methods professor cheng-ping wen's 164 drugs for the treatment of rheumatoid arthritis symptoms,frequency,the association rules between drugs and between drugs and symptoms and clustering analysis of symptoms were performed and their experience of the treatment of this disease was refined.Results According to the data analysis,the most frequently used drugs in prescription were honeysuckle,centipede and licorice.Among the traditional Chinese medicine pairs,centipede,Tripterygium wilfordi and Dodder had the highest degree of support.The highest degree of promotion was Fenji and Coix seed,and the highest degree of confidence was Cuscuta,Yam and Cuscuta.ConclusionAccording to the pathogenesis characteristics of spleen deficiency combined with dampness,toxicity and blood stasis in RA patients,Professor Wen Chengping's treatment of RA mainly focuses on detoxification and dredging collaterals,and pays attention to strengthening spleen clearing damp and protecting spleen and stomach.This method has certain clinical significance for the treatment of rheumatoid arthritis.
