摘 要:目的:评价Ligs关节韧带数字体查仪测量麻醉下膝关节前向松弛度的可靠性,并与KT-1000测量结果进行对比。方法:2022年2月1日至5月31日期间就诊于本院运动医学科诊断为单侧急性前交叉韧带(anterior cruciate ligament,ACL)完全损伤患者为受试者。受试者在手术室麻醉后,采用Ligs关节韧带数字体查仪和KT-1000进行双膝关节前向松弛度的测量,计算患侧与健侧胫骨前移侧-侧差值。采用组内相关系数(ICC)分析不同方法测量膝关节前向松弛度的观察者间信度和复测信度,并比较Ligs装置与KT-1000在不同拉力(67 N、89 N、133 N)下所得膝关节前向不稳定的测量结果。结果:本研究共纳入54例患者,平均年龄27.4±6.3岁。Ligs测量膝关节前向松弛度的观察者间信度为0.78~0.80,复测信度为0.83~0.86。67 N拉力时,KT-1000测量的胫骨前移侧-侧差值为5.7±1.4 mm,Ligs测量结果为5.9±1.5 mm,两者之间差异无统计学意义(P=0.876)。89 N和133 N测量时,两种测量方法的结果差异也无统计学意义。结论:对于ACL完全损伤患者,Ligs关节韧带数字体查仪测量麻醉下膝关节前向松弛度的观察者间信度和复测信度均较高。此外,在不同拉力(67 N、89 N、133 N)下测量ACL损伤患者的胫骨前向松弛度时,Ligs关节韧带数字体查仪和KT-1000的测量结果一致。Objective To evaluate the reliability of Ligs joint ligament digital physical examination instrument in measuring anterior knee laxity under anesthesia,and compare it with KT-1000 measurement results. Methods Patients diagnosed as complete unilateral acute anterior cruciate ligament(ACL)injury in the Department of Sports Medicine of our hospital between February 1stand May 31stwere selected. After they were anesthetized in the operating room,they were measured the anterior laxity of both knees using the Ligs joint ligament digital physical examination instrument and KT-1000,respectively,and the side-to-side difference of anterior tibia between the affected and healthy side was calculated. The intraclass correlation coefficient(ICC) was used to analyze the inter-observer reliability and test-retest reliability of different devices measuring the anterior knee laxity. Moreover,the anterior instability of knee joint was measured using the Ligs device and KT-1000 under different pulling forces(67 N,89 N and 133 N). Results A total of 54 patients were included in this study,with an average age of 27.4 ± 6.3 years old. The inter-observer reliability and the test-retest reliability of Ligs device measuring anterior knee laxity was 0.78~0.80 and 0.83~0.86. At 67 N tension,the average sideto-side difference of tibia anterior displacement was 5.7 ± 1.4 and 5.9 ± 1.5 mm according to the KT-1000 and Ligs measurement,and there was no significant difference between them(P=0.876). Moreover,no significant differences were found in the above value using two different methods when the pulling forces were 89 N and 133 N respectively. Conclusion For patients with complete ACL injury,the inter-observer reliability and test-retest reliability of anterior knee joint laxity under anesthesia are high measured by using the Ligs digital device. Moreover,the anterior tibial laxity of such patients is consistent using the Ligs and the KT-1000 devices under different tension conditions(67 N,89 N and 133 N).
