摘  要:目的采用完全弗氏佐剂诱导慢性炎症痛(chronic inflammatory pain,CIP)大鼠模型,探讨右美托咪定(dexmedetomidine,Dex)对CIP大鼠疼痛及脊髓高迁移率族蛋白B1(high mobility group protein B1,HMGB1)表达的影响。方法48只大鼠随机分为对照组(不干预)、假手术组(左后足趾皮下注射生理盐水)、CIP组(左后足趾皮下注射完全弗氏佐剂)、Dex组(左后足趾皮下注射完全弗氏佐剂+鞘内注射Dex)各12只。造模后观察各组大鼠行为学表现,造模前、鞘内注射前1 h及注射后2、24 h检测各组大鼠左后肢热缩足反射潜伏期(paw withdrawal thermal latency,PWTL)及机械性缩足反射阈值(mechanical withdrawal threshold,MWT),注射后2、24 h采用ELISA法检测脊髓TNF-α、IL-1β水平,注射后24 h采用实时荧光定量PCR法检测脊髓HMGB1、Toll样受体4(Toll-like receptor 4,TLR4)、核因子-κB(nuclear factor-κB,NF-κB)mRNA相对表达量,采用Western blot法检测脊髓HMGB1、TLR4、NF-κB蛋白相对表达量。结果(1)造模后对照组、假手术组大鼠健康状况均良好;CIP组大鼠饮食饮水基本正常,但活动量减少,患侧逐渐出现典型的自发痛觉过敏体征;Dex组大鼠一般状况及患肢运动行为较CIP组均有所改善。(2)造模前,4组PWTL、MWT比较差异均无统计学意义(P>0.05);注射前1 h,Dex组、CIP组PWTL、MWT均低于假手术组、对照组(P<0.05),Dex组与CIP组比较差异均无统计学意义(P>0.05);注射后2、24 h,CIP组PWTL、MWT均低于Dex组、假手术组、对照组(P<0.05),Dex组均低于假手术组、对照组(P<0.05);假手术组各时间点PWTL、MWT与对照组比较差异均无统计学意义(P>0.05)。CIP组注射前1 h、注射后2 h、注射后24 h PWTL、MWT均低于造模前(P<0.05);Dex组注射前1 h、注射后2 h、注射后24 h PWTL、MWT均低于造模前(P<0.05),注射后2 h均高于注射前1 h、注射后24 h(P<0.05)。(3)CIP组注射后2、24 h脊髓TNF-α、IL-1β水平均高于假手术组、对照组(P<0.05),假Objective To investigate the influences of dexmedetomidine(Dex)on pain and expression of spinal cord high mobility group protein B1(HMGB1)in complete Freund\s adjuvant(CFA)induced chronic inflammatory pain(CIP)rat models.Methods Forty-eight rats were randomly divided into normal group(no intervention),sham-operation group(subcutaneous injection of saline in the left rear toe),CIP group(subcutaneous injection of CFA in the left rear toe),and Dex group(subcutaneous injection of CFA in the left rear toe+intrathecal injection of Dex),with 12rats in each group.After modeling,the behaviors of rats were observed in each group.The paw withdrawal thermal latency(PWTL)and mechanical withdrawal threshold(MWT)were detected before modeling,1hbefore intrathecal injection,and 2and 24h after injection,respectively.The relative expressions of tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β)in the spinal cord were detected by ELISA 2and 24hafter injection.The levels of HMGB1,Toll-like receptor 4(TLR4)and nuclear factor-κB(NF-κB)mRNAs were detected by real-time fluorescence quantitative PCR 24hafter injection.The relative expressions of HMGB1,TLR4and NF-κB proteins were detected by Western blot.Results The rats in control group and sham-operation group were healthy after modeling.The rats in CIP group had normal diet and water intake,but their activity level worsened,and typical signs of spontaneous hyperalgesia gradually appeared on the affected side.The general condition and motor behavior of affected limb of rats in Dex group were improved.Before modeling,there were no significant difference in PWTL and MWT among four groups(P>0.05).PWTL and MWT were lower in Dex group and CIP group than those in sham-operation group and control group 1hbefore injection(P<0.05),and showed no significant differences between Dex group and CIP group(P>0.05).PWTL and MWT were lower 2and 24hafter injection in CIP group than those in Dex group,sham-operation group and control group(P<0.05),and lower in Dex group than those in sham-operat
