载脂蛋白A1对霍奇金淋巴瘤预后的影响

摘　　要：目的 探讨霍奇金淋巴瘤(HL)患者血清载脂蛋白A1水平的变化及其临床意义。方法 收集中国医科大学附属盛京医院2013年1月至2017年1月初治的HL患者88例,同期90例健康成人作为对照,分析载脂蛋白A1和载脂蛋白B的水平。结果 HL组载脂蛋白A1水平(1.04±0.25)ng/L低于正常对照组(1.25±0.20)ng/L。载脂蛋白B较正常人无明显变化。当疾病缓解时载脂蛋白A1水平可升高(1.30±0.17)ng/L,疾病进展时载脂蛋白A1水平降低(0.66±0.22)ng/L。多因素分析表明载脂蛋白A1减少是霍奇金淋巴瘤的独立危险因素。高载脂蛋白A1组与低载脂蛋白A1组的Kaplan-Meier生存分析显示高载脂蛋白A1组有更高的生存率和更长的无进展生存期。结论 载脂蛋白A1是霍奇金淋巴瘤有效的肿瘤负荷生物标记物和预后因素。Objective To determine the change and clinical significance of serum level Apo A1 in Hodgkin's lymphoma patients.Methods A restrospective review of the medical records of 88 patients admitted to Shengjing Hospital of China Medical University,between Jan.2013 and Jan.2017 for hodgkin's lymphoma.Ninety healthy adult people were used as controls.The serum levels of Apo A1 and Apo B in patients were retrospectively analyzed.Results Our study showed that value of Apo A1 in the patient group(1.04±0.25)ng/L was lower than that in the control group(1.25±0.20)ng/L.Apo B was no significant change between patient group and control group.Apo A1 was elevated when the disease was in remission(1.30±0.17)ng/L,and decreased after disease in progression(0.66±0.22)ng/L.According the result of multivariate analysis Apo A1 reduction become the independent risk factors of HL.On the basis of Kaplan-Meier survival analysis between high Apo A1 subgroup and low Apo A1 subgroup,we found higher Apo A1 patienta had longer OS rate and PFS.Conclusion Apo A1 is a useful biomarker of tumor burden and a prognostic factor of hodgkin's lymphoma.
