摘 要:目的 分析广西沿海地区不同孕期的孕妇甲状腺激素指标变化,为临床干预提供指导。方法 选取2020年1—12月在广西沿海5个县区的县医院进行定期孕期保健的健康孕妇301例为研究对象,另收集2020年在同一家医院体检的健康非妊娠妇女302例作为对照组。采用进口罗氏E602电化学免疫发光分析仪对沿海地区正常孕妇及非妊娠妇女的甲状腺激素水平进行检测。结果 与正常非妊娠妇女比较,妊娠期孕妇TSH、FT_(3)、FT_(4)水平较低,而Tg水平较高,差异有统计学意义(P均<0.05)。与孕早期比较,孕中、晚期孕妇TSH、Tg水平明显升高,且随着孕周增加逐渐升高(P<0.05);与孕早期比较,孕中、晚期孕妇FT_(3)、FT_(4)水平明显降低,且随着孕周增加逐渐降低(P<0.05)。结论 正常孕妇的甲状腺激素水平不同于非妊娠妇女,孕早中晚期也存在明显差异,应加强孕期甲状腺功能监测。Objective To analyze the present status and trend of iodine deficiency disorders(IDD) in coastal areas of Guangxi, and to provide the evidence for prevention and treatment of the disease.Methods From January to December 2020,301 pregnant women in five county hospitals along the coast of Guangxi were selected, other 302 non-pregnant women in the same period were selected as the control group.The thyroid hormone levels of normal pregnant women and non-pregnant women in coastal areas were detected by imported Roche E602 electrochemical immunoluminescence analyzer.Results Compared with non-pregnant women, the TSH、FT_(3)、FT_(4)levels of pregnant women were lower, but the level of Tg was higher, with statistically significant differences(all P<0.05).Compared with early pregnancy, the levels of TSH and Tg of pregnant women in middle and late pregnancy were increased significantly, and with the prolongation of pregnancy of pregnant women, the TSH and Tg levels gradually increased(P<0.05);While the levels of FT_(3)and FT_(4)of pregnant women in middle and late pregnancy were decreased significantly, and with the prolongation of pregnancy of pregnant women, the TSH and Tg levels gradually decreased(P<0.05).Conclusion There are differences in the thyroid hormone levels in normal pregnant woman and non-pregnant women, there are also significant differences in early, middle and late pregnancy, and the monitoring of thyroid function markers should be strengthened in pregnancy.
