多模态影像组学辅助诊断早期乳腺癌的效能评价

摘　　要：目的 探讨基于超声、X线钼靶、MRI及CT的多模态影像组学对早期乳腺癌的诊断价值.方法 对收治入院的75例早期乳腺癌患者行超声、X线钼靶、MRI及CT扫描,将扫描结果与术后病理结果进行比对,并分析其诊断效果.结果 1)超声:可见血流信号,有细点钙化;2)X线钼靶:有结节影及钙化灶(簇状泥沙样钙化),钙化灶边缘模糊有毛刺;3)MRI:出现局灶性、导管样及段样、肿块化等强化,肿块边缘不规则有毛刺;4)CT:出现肿块或结节,部分边缘较为光滑,另有小部分较为模糊,出现钙化或肿块肿大.与术后病理比对,诊断的准确性、敏感度、特异度超声为76.00%、80.00%、60.00%,X线钼靶为77.33%、81.67%、60.00%,MRI为77.33%、80.00%、66.66%,CT为81.33%、83.33%、86.67%,联合诊断为90.67%、91.67%、86.67%.结论 超声、X线钼靶、MRI及CT均为乳腺癌早期诊断的常见方法,4种方法有各自的特点和优势,联合诊断可提升早期诊断的效能.Objective To explore the value of multimodal imaging based on ultrasound,X-ray molybdenum target,MRI and CT enhancement in early diagnosis of breast cancer.Method 75 cases of early breast cancer patients underwent ultrasound,X-ray mammography,MRI and CT enhanced scan,postoperative pathological results were compared to analyze the diagnostic effect of each method.Results 1)Ultrasound:blood flow signal can be seen,with fine calcification;2)X-ray molybdenum target:nodules and calcification foci(cluster sediment calcification),and the edge of calcification foci fuzzy burr;3)MRI:there were focal,catheter-like and segmental enhancement,and mass enhancement,with irregular edges and burrs;4)CT:there were lumps or nodules,some edges are smooth and others are fuzzy,calcification or mass enlargement.In terms of diagnostic accuracy,sensitivity and specificity,ultrasound were 76.00%,80.00%,60.00%,X-ray molybdenum target was 77.33%,81.67%,60.00%,MRI was 77.33%,80.00%,66.66%,CT was 81.33%,83.33%,86.67%.Combined diagnosis was 90.67%,91.67%and 86.67%.Conclusion Ultrasound,X-ray molybdenum target,MRI and CT enhancement are common methods for the early diagnosis of breast cancer,with their own characteristics and advantages.The combined diagnosis of the four methods can improve the efficiency of early diagnosis.
