补肾活血汤对膜性肾病大鼠足细胞的保护作用

摘  要:目的:利用被动型Heymann肾炎(passive Heymann nephritis,PHN)模型探讨补肾活血汤对血清IL-6、TNF-α的影响及对大鼠足细胞损伤的保护作用。方法:利用兔抗大鼠Fx1A抗血清建立PHN模型,30只SD大鼠随机分为对照组、模型组和中药组。中药组PHN大鼠予补肾活血汤,1次/d,共28 d。治疗前后检测24 h尿蛋白定量;ELISA检测血清IL-6和TNF-α水平;光镜碘酸六胺银染色(periodic acid-silver metheramine,PASM)及电镜观察肾脏病理改变,实时定量PCR(real time quantitative PCR,RT-qPCR)及免疫荧光检测肾组织Podocin的mRNA及蛋白表达。结果:模型组肾小球足细胞足突广泛融合,基底膜增厚伴上皮侧电子致密物沉积,中药组上述病理损伤减轻。与对照组相比,模型组24 h尿蛋白上升,肾脏Podocin的mRNA及蛋白表达下降,血清IL-6、TNF-α水平升高(P<0.01)。与模型组相比,中药治疗后24 h尿蛋白、IL-6、TNF-α水平下调,足细胞Podocin表达得到部分恢复(P<0.05)。结论:补肾活血汤可降低PHN大鼠24 h尿蛋白,减轻肾脏病理损伤,恢复足细胞Podocin的表达,补肾活血汤的治疗机制可能与下调血清IL-6和TNF-α的水平相关。Objective:To explore the therapeutic effect of Bushen Huoxue Decoction(BSHX)on podocyte injury and the influence on the levels of serum IL-6 and TNF-αin rat with passive Heymann nephritis(PHN).Methods:The PHN model induced by rabbit anti-rat Fx1A serum,30 rats were randomly divided into control,PHN model and traditional Chinese medicine(TCM)group.The rats with PHN in TCM group were treated with BSHX once daily for 28 days.The 24-hour urinary protein excretion was measured before and after treatment.The serum levels of IL-6 and TNF-αwere tested by ELISA.The renal pathology with periodic acid-silver metheramine(PASM)stain as well as the electron microscopic for ultrastructure change was evaluated.The mRNA and protein of Podocin were detected by real-time quantitative PCR(RT-qPCR)and immunofluorescence in renal tissue.Results:Typical pathological features of PHN including extensive fusion of podocyte foot process,diffused thickened of glomerular basement membrane with subepithelial electron dense deposits were found in PHN model group while the injury was alleviated in TCM group.Compared with the control group,proteinuria excretion as well as the levels of serum IL-6 and TNF-αwere increased(P<0.01),the expression of Podocin was decreased in rats with PHN significantly(P<0.01).Compared with the PHN model group,the levels of proteinuria,IL-6 and TNF-αwere decreased significantly(P<0.05),while the expression of Podocin partially restored after treatment with BSHX.Conclusion:BSHX can decrease the 24-hour urinary protein,alleviate renal pathological injury,and restore the expression of Podocin in PHN rat.The therapeutic mechanism of BSHX may be related to reducing serum level of IL-6 and TNF-α.
