肿痛散对毒蛇咬伤肢体肿痛及炎症因子、血液流变学的影响

摘　　要：目的 探讨肿痛散对毒蛇咬伤后肢体肿痛的临床疗效及其对炎症因子、血液流变学的影响。方法 选取2019年1月至2021年12月在我院就诊的毒蛇咬伤后肢体肿痛患者120例,按随机数字表法分为治疗组和对照组各60例。对照组在西医常规治疗基础上采用硫酸镁湿外敷治疗,治疗组在西医常规治疗基础上采用肿痛散(我科协定方)湿外敷治疗。连续治疗7 d后,比较两组伤肢肿痛消退效果以及治疗前后炎症因子水平、血液流变学指标变化情况。结果 治疗7 d后,治疗组的肿痛消退有效率为96.67%,对照组为73.33%,两组比较差异有统计学意义(P<0.05);治疗组的疼痛程度评分(VAS)、肢体肿胀消退情况改善均优于对照组,差异均有统计学意义(均P<0.05)。治疗后,两组患者的血浆黏度(PSV)、全血高切黏度(HSV)、全血低切黏度(LSV)、血沉(ESR)等血液流变学指标及C反应蛋白(CRP)、肿瘤坏死因子-a(TNF-a)、白细胞介素-6(IL-6)等炎性因子指标均较治疗前改善,且治疗组各项指标改善优于对照组,差异均有统计学意义(均P<0.05)。结论 肿痛散能加速毒蛇咬伤肿胀疼痛症状消退,降低炎症反应,改善血液流变学指标。Objective To investigate the clinical effect of Zhong Tong Xian on limb swelling and pain after poisonous snake bite and its effect on inflammatory factors and hemorheology.Methods A total of 120 patients with limb swelling and pain after venomous snake bite in our hospital from January 2019 to December 2021 were selected and divided into treatment group and control group according to random number table method(60 cases in each group).On the basis of conventional treatment in western medicine,the control group was treated with wet external application of magnesium sulfate while the treatment group was treated with wet external application of Zhong Tong Xian formula agreed by our department of medicine.The clinical efficacy,inflammatory factor levels and hemorheology indexes of the two groups were compared after 7 days of continuous treatment.Results After 7 days of treatment,the total effective rate in the treatment group was 96.67%,which was higher than that in the control group(73.33%)(P<0.05).The treatment group was superior to the control group in Vas and limb swelling after treatment,hemorheological indexes such as plasma viscosity(PSV),whole blood high shear viscosity(HSV),whole blood low shear viscosity(LSV),erythrocyte sedimentation rate(ESR)and inflammatory factors such as C-reactive protein(CRP),tumor necrosis factor-A(TNF-A)and interleukin-6(IL-6)in 2 groups decreased compared with those before treatment.The improvement of each index in treatment group were significantly better than that in control group(all P<0.05).Conclusion The Zhong Tong Xian can quickly promote the regression of the swelling and pain symptoms of snake bite patients,reduce the inflammatory response and improve the hemorheology index.
