“瓦合”法搭建自体肋软骨支架在鼻综合整形术中的应用

摘　　要：目的:探讨“瓦合”法搭建自体肋软骨支架,在鼻综合整形术中的应用效果。方法:选择2018年1月-2020年12月43例鼻综合整形就医者为研究对象,将就医者软骨取下后,沿其长轴斜型切开并雕刻成四片宽度相同、长度不同的薄片,用“瓦合”法搭建肋软骨支架,四个薄片交错瓦合,互相支撑,形成稳定的三角形结构,以此增加鼻背长度,抬高鼻尖,并覆盖软骨膜及腱膜组织,改善鼻尖外形。结果:43例就医者均一期愈合。随访6个月~1年,其中3例鼻尖出现轻度偏斜,用耳软骨进行调整后,效果满意,其余就医者鼻外形良好,鼻尖微翘、纤细,鼻背高度满意。结论:用“瓦合”法搭建自体肋软骨支架行鼻综合整形术,可以明显延长鼻背、抬高鼻尖并改善鼻部美学形态,支架具有良好的支撑力和稳定性,在鼻整形领域应用前景广阔。Objective To investigate the clinical effect of constructing autogenous costal cartilage stent by"tile"method in rhinoplasty.Methods From January 2018 to December 2020,43 patients with comprehensive rhinoplasty were selected as the research objects.between the cartilage removed,along its long axis inclined cut and carved into four pieces of the same width,length,different chip,using"close"structures,rib cartilage,the four chip staggered together,support each other,form a stable triangle structure,thus increasing the length of the dorsum,push up the tip,The perichondrium and aponeurosis were covered to improve the shape of the nasal tip.Results All the 43 patients healed in the fi rst stage.During the follow-up period of six months to one year,3 cases of nasal tip appeared slight deviation,and the eff ect was satisfactory after adjustment with ear cartilage,the other patients had a good nose appearance,the tip of the nose slightly warped,slim,and the nasal back was highly satisfactory.Conclusion The autogenous rib cartilage stent can obviously prolong the nasal back,elevate the nasal tip and improve the aesthetic shape of the nose.It has a promising application prospect in rhinoplasty.
