容量移位和容量替代技术在乳腺癌保乳术中的应用及对预后的影响

摘　　要：目的:探讨容量移位和容量替代技术在乳腺癌保乳手术中应用效果及对预后的影响。方法:选择152例行容量移位或容量替代技术整形保乳术治疗的乳腺癌患者,对其手术指标、术后6个月时乳房美学效果评价结果及预后等资料进行回顾性整理分析。结果:患者术中采取容量移位者97例(63.82%)、容量替代者55例(36.18%);行容量替代的患者病灶最大径大于行容量移位患者,差异有统计学意义(P<0.05);行容量替代的患者术中出血量及切除标本体积大于行容量移位患者,差异有统计学意义(P<0.05),切口长度及手术时间比较差异无统计学意义(P>0.05);行容量替代的患者乳房对称性、皮肤弹性、质地与弹性、凹陷程度、手术瘢痕、乳头横向移位距离、乳头纵向移位距离、患者满意度等项目评分均低于容量移位患者,差异有统计学意义(P<0.05);术后18~36个月,患者生存率均为100.00%,无死亡患者,转移率与复发率行容量移位的患者分别为6.19%、2.06%,行容量替代的患者分别为7.27%、1.82%,差异无统计学意义(P>0.05)。结论:容量移位整形保乳术治疗乳腺癌较容量替代术具有更好的美学效果,且对患者预后无不良影响,不会增加肿瘤复发及转移风险性。Objective To explore the aesthetic eff ect and prognosis of volume displacement and volume replacement technique in breast conserving surgery for breast cancer.Methods 152 patients with breast cancer on volume displacement and volume replacement technique oncoplastic breast conserving surgery,the clinical data of 152 patients with breast cancer who oncoplastic breast conserving surgery were collected,the general data of the patients,surgical indicators,breast aesthetic eff ect evaluation results and prognosis at 6 months after surgery were retrospectively analyzed.Results The 97 cases patients(63.82%)underwent volume displacement and 55 cases patients(36.18%)underwent volume replacement technique oncoplastic breast conserving surgery.The maximum diameter of the lesion in patients with volume replacement was larger than that the patients with volume displacement,the diff erence was statistically signifi cant(P<0.05).The intraoperative blood loss and volume of excised specimen in patients with volume replacement was larger than that the patients with volume displacement,the diff erence was statistically significant(P<0.05),there was no significant difference in incision length and operation time(P>0.05).The breast symmetry,skin elasticity,texture and elasticity,degree of sag,surgical scar,transverse displacement distance of the nipple,longitudinal displacement distance of the nipple,patient satisfaction in patients with volume replacement was lower than that the patients with volume displacement,the diff erence was statistically signifi cant(P<0.05).The survival rate of all patients at 18-36 months after surgery was 100.00%,and no patients died,the metastatic rate and recurrence rate of patients with volume displacement were 6.19%and 2.06%,patients with volume displacement were 7.27%and 1.82%,there was no significant diff erence in incision length and operation time(P>0.05).Conclusion The aesthetic eff ect of volume displacement technique better than that of volume replacement technique oncoplastic breast conservin
