耳前大S形切口在腮腺浅叶良性肿瘤切除术中的应用及美学效果观察

摘　　要：目的:探析耳前大S形切口在腮腺浅叶良性肿瘤切除术中的应用及美学效果。方法:选取2018年1月-2021年1月诊治的110例腮腺浅叶良性肿瘤患者为研究对象,所有患者均行腮腺浅叶切除术,其中采用耳前大S形切口(观察组)和传统切口(对照组)各55例,比较两组围术期指标、术后并发症及切口美观满意度,并于术后随访6个月,观察有无肿瘤复发情况。结果:所有患者均顺利完成手术。与对照组比较,观察组切口长度较长,术中出血量、面神经解剖支数较少,创面愈合时间较短,手术总有效率及术后6个月切口美观满意度视觉模拟量表(Visual analogue scale,VAS)评分较高,术后涎瘘及总并发症发生率较低,差异均有统计学意义(P<0.05);两组手术时间、住院时间、术后3个月切口美观满意度VAS评分及术后6个月肿瘤复发率比较差异无统计学意义(P>0.05)。结论:在腮腺浅叶良性肿瘤切除术中采用耳前大S形切口能减少术中出血量和面神经分支解剖数,切口创面愈合快,手术效果好,术后并发症少,切口美观度高,值得临床推广。Objective The aim of this study was to explore the the application and aesthetic eff ect of large S-shaped incision in the resection of benign tumors in the superfi cial lobe of parotid gland.Methods A total of 110 patients with superfi cial parotid benign tumors were selected between January 2018 and January 2021.All were treated with superfi cial parotidectomy.According to the surgical approach,the patients enrolled were divided into observation group(55 cases,preauricular large S-shaped incision)and control group(55 cases,traditional incision).The perioperative indexes,postoperative complications and satisfaction with incision appearance were compared between the two groups.Six months of follow-up was carried out after surgery to observe the situation of tumors recurrence.Results All patients successfully completed the surgery.Compared with control group,observation group had longer incision length,less intraoperative blood loss,fewer anatomical branches of facial nerve,shorter wound healing time,higher eff ective rate of surgery and the visual analog scale(VAS)score of incision satisfaction with appearance at 6 months after the operation,the incidence of postoperative salivary fi stula and total incidence of complications was lower,and the diff erences were statistically signifi cant(P<0.05).There was no signifi cant diff erence between the 2 groups in the time of operation,the length of hospital stay,the VAS score at 3 months after operation,and tumor recurrence rate at 6 months after surgery(P>0.05).Conclusion Resection of superfi cial parotid benign tumor through preauricular large S-shaped incision can reduce intraoperative blood loss and anatomical number of facial nerve branches,and the wound healing quickly,with good eff ect,few complications and high aesthetics.
