以辨病位病性为核心探讨慢性前列腺炎/慢性盆腔疼痛综合征的针灸诊疗思路

摘　　要：以辨病位、辨病性为核心探讨针灸治疗慢性前列腺炎/慢性盆腔疼痛综合征的诊治思路。在临床诊疗过程中,以“理、法、方、穴、术”为纲,注重明辨病位、病性,基于本病初期“标实”为主,病久“本虚”为主的病机特点,初期重在“疏通冲任气血、化瘀清浊”,以任脉、足厥阴肝经、足太阴脾经腧穴为主,主取中极、会阴、气冲、太冲、蠡沟、三阴交、中髎穴;病久重在“调理脏腑,疏肝理脾补肾”,以足太阴脾经、足少阴肾经及任脉腧穴为主,主取三阴交、太溪、太冲、足三里、关元、气海穴。据病程不同通调结合,配合经络腧穴诊察及适宜的刺灸方法。This paper explored the idea of diagnosis and treatment of chronic prostatitis/chronic pelvic pain syndrome by acupuncture and moxibustion based on differentiating disease location and property.In the diagnosis and treatment process,“principles,methods,formulas,acupoints and techniques”are taken as the guidance,and it is critical to differentiate the disease location and property.The pathomechanism is characterized by“branch excess”in the early stage and“root deficiency”in the late stage.Therefore,it is important to dredge the chong(冲)and ren(任)mai,and unblock qi and blood stasis in the early stage.The acupoints of Conception Vessel,the Foot Jueyin(厥阴)Liver channel and Foot Taiyin(太阴)Spleen channel are mainly used,such as Zhongji(CV3),Huiyin(CV1),Qichong(ST30),Taichong(LR3),Ligou(LR5),Sanyinjiao(SP6)and Zhongliao(BL33).In the late stage,it is important to regulate zang-fu(脏腑)organs,sooth the liver,invigorate the spleen and tonify the kidney.The acupoints of Conception Vessel,Foot Shaoyin(少阴)Kidney channel and Foot Taiyin(太阴)Spleen channel are mainly used,such as Sanyinjiao(SP6),Taixi(KI3),Taichong(LR3),Zusanli(ST36),Guanyuan(CV4)and Qihai(CV6).It is an effective acupuncture and moxibustion treatment scheme by combining the channels and collaterals regulation and invagoration,and selecting the proper acupoint and needling or moxibustion method according to the disease course.
