对于社区获得性肺炎多重PCR联检试剂性能评价和临床应用价值探究

摘　　要：目的:考察及评价多重PCR联检试剂在社区获得性肺炎(CAP)的临床诊断性能和应用价值。方法:同时用多重PCR联检试剂和肺炎支原体单检试剂检测采集727例有呼吸道感染症状的患者咽拭子样本,比较分析两种试剂的灵敏度、结果一致性和相关性,对于重点关注的患者进行治疗监测。结果:727例样本PCR联检试剂和单检试剂检测结果的阳性一致性为97.69%,阴性一致性为99.66%,总一致性为99.31%,其中阴阳性不符合的5例标本,两种检测试剂诊断一致非常高(κ=0.9765,P<0.0001);在127例肺炎支原体阳性样本中,联检试剂检出了6例流感病毒或呼吸道合胞病毒混合感染病例,在疗效监测中发现单一病原体检测结果对于用药指导信息可能不够充足。结论:多重PCR技术可以在实验检测中一次性检测多种病原体,且具备单检试剂的检测性能,对于多重病原体感染有着一定的临床意义。Objective:To investigate and evaluate a multiplex PCR reagent's in clinical diagnostic performance and application value for community-acquired pneumonia(CAP).Methods:the pharyngeal swab samples of 727 patients with respiratory tract infection were collected by simultaneous detection of multiple PCR The sensitivity,specificity accuracy,of the two reagents were compared and analyzed.The treatment process of co-infection patients was recorded.Results:The positive consistency of multiplex PCR reagent and single test reagent was 97.69%,the negative consistency was 99.66%,and the total consistency was 99.31%(κ=0.9765,P<0.0001);Among 127 positive samples of Mycoplasma pneumoniae,6 cases of mixed infection of influenza virus or respiratory syncytial virus were detected by the multiplex PCR reagent.The detection results of single pathogen may not be sufficient for medication guidance.Conclusion:Multiplex PCR technology has the same performance as the single detection reagent and could detect various pathogens in one experiment,which is the essential clinical significance for pathogen co-infections.
