便塞通合剂对气阴两虚证老年慢性功能性便秘患者的临床疗效

摘　　要：目的探讨便塞通合剂对气阴两虚证老年慢性功能性便秘患者的临床疗效。方法118例患者随机分为对照组(58例)和观察组(60例),对照组采用芪黄通秘软胶囊,观察组采用便塞通合剂,疗程6周。检测临床疗效、CSBM、便秘症状评分、气阴两虚证评分、PAC-QOL评分、肠道菌群(双歧杆菌、乳酸杆菌、类杆菌、大肠杆菌、肠球菌)数量和代谢物(SCFAs、胆汁酸)、血清脑肠肽(5-HT、NPY、MTL、SP、VIP)、复发率、安全性指标变化。结果观察组愈显率高于对照组(P<0.05)。治疗后,2组CSBM及双歧杆菌、乳酸杆菌、类杆菌数量增加(P<0.05),大肠杆菌、肠球菌数量减少(P<0.05),便秘症状评分、气阴两虚证评分、PAC-QOL评分、NPY、VIP降低(P<0.05),肠道菌群代谢物、5-HT、MTL、SP升高(P<0.05),以观察组更明显(P<0.05)。随访3个月,2组复发率比较,差异无统计学意义(P>0.05),并且均未发现严重不良反应。结论便塞通合剂可安全有效地调节气阴两虚证老年慢性功能性便秘患者“脑-肠-菌”轴,改善胃肠动力,减轻便秘症状,恢复自主排便,提高生活质量。AIM To explore the clinical effects of Biansaitong Mixture on patients with elder chronic functional constipation due to dual deficiency of Qi and Yin.METHODS One hundred and eighteen patients were randomly assigned into control group(58 cases)for 6-week administration of Qihuang Tongmi Soft Capsules,and observation group(60 cases)for 6-week administration of Biansaitong Mixture.The changes in clinical efficacy,CSBM constipation symptom score,dual deficiency of Qi and Yin score,PAC-QOL score,intestinal flora(Bifidobacterium,Lactobacillus,Bacteroid,Escherichia coli,Enterococcus)counts and metabolites(SCFAs,bile acids),serum brain gut peptides(5-HT,NPY,MTL,SP,VIP),recurrence rate and safety indices were detected.RESULTS The observation group demonstrated higher good improvement rate than the control group(P<0.05).After the treatment,the two groups displayed elevated CSBM and Bifidobacterium,Lactobacillus,Bacteroid counts(P<0.05),reduced Escherichia coli,Enterococcus counts(P<0.05),decreased constipation symptom score,dual deficiency of Qi and Yin score,PAC-QOL score,NPY,VIP(P<0.05),and increased intestinal flora metabolites,5-HT,MTL,SP(P<0.05),especially for the observation group(P<0.05).At 3-month follow-up,no significant difference in recurrence rate was found between the two groups(P>0.05)without serious adverse reaction.CONCLUSION For the patients with elder chronic functional constipation due to dual deficiency of Qi and Yin,Biansaitong Mixture can safely and effectively regulate‘brain-intestine-bacteria’axis,improve gastrointestinal motility,relieve constipation symptom,restore spontaneous defecation,and enhance life quality.
