三阴性乳腺癌血清外泌体和组织中lncRNA TINCR的表达及临床意义

摘　　要：目的探讨长链非编码RNA(lncRNA)组织分化诱导非蛋白编码(TINCR)在三阴性乳腺癌(TNBC)患者血清外泌体和组织中的表达情况,探讨其表达水平与TNBC发生、发展的关系。方法选取该院2017年1月至2019年1月收治的83例TNBC患者作为TNBC组,再选取同期就诊的62例非TNBC的乳腺癌患者作为非TNBC组,60例体检健康者作为对照组,检测机体血清外泌体和癌组织中lncRNA TINCR表达情况,分析lncRNA TINCR在血清外泌体和癌组织中表达的相关性,分析血清外泌体和癌组织中lncRNA TINCR表达与患者临床特征及预后的关系,绘制受试者工作特征(ROC)曲线分析血清外泌体lncRNA TINCR表达对TNBC的诊断价值。结果TNBC组血清外泌体中lncRNA TINCR表达水平高于非TNBC患者和对照组(P<0.05),非TNBC组血清外泌体中lncRNA TINCR表达水平高于对照组(P<0.05);TNBC患者癌组织中lncRNA TINCR表达水平明显高于TNBC患者癌旁组织和非TNBC患者癌组织(P<0.05);Pearson相关性结果显示,癌组织与血清外泌体lncRNA TINCR表达水平呈正相关(P<0.05);血清外泌体和癌组织中lncRNA TINCR表达水平升高与TNBC患者肿瘤T分期、N分期和分级有关(P<0.05);ROC曲线结果显示,血清外泌体lncRNA TINCR表达水平诊断TNBC曲线下面积为0.772;截止随访时间TNBC患者总生存率为85.54%,共死亡12例,其中癌组织lncRNA TINCR高表达患者死亡率高于低表达患者(P<0.05)。死亡患者癌组织和血清外泌体lncRNA TINCR表达水平均高于生存患者(P<0.05)。结论血清外泌体和癌组织lncRNA TINCR在TNBC患者中表达上调,对TNBC诊断和病情判断具有一定的应用价值。Objective To investigate the expression of long non-coding RNA(lncRNA)tissue differentiation induced non-protein coding(TINCR)in serum exosomes and tissues of patients with triple negative breast cancer(TNBC),and to explore the relationship between its expression level and the occurrence and development of TNBC.Methods A total of 83 patients with TNBC admitted to the hospital from January 2017 to January 2019 were selected as the TNBC group,62 non-TNBC breast cancer patients were selected as the non-TNBC group,and 60 healthy physical examination subjects were selected as the control group.The expression of lncRNA TINCR in serum exosomes and cancer tissues was detected,the correlation between the expression of lncRNA TINCR in serum exosomes and cancer tissues was analyzed,and the relationship between the expression of lncRNA TINCR in serum exosomes and cancer tissues and the clinical characteristics and prognosis of patients was analyzed,and the receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of lncRNA TINCR expression in serum exosomes for TNBC.Results The expression level of lncRNA TINCR in serum exosomes of TNBC group was higher than that of non-TNBC group and control group(P<0.05).The expression level of lncRNA TINCR in serum exosomes of non-TNBC group was higher than that of control group(P<0.05).The expression level of lncRNA TINCR in cancer tissues of TNBC patients was significantly higher than that in adjacent tissues of TNBC patients and cancer tissues of non-TNBC patients(P<0.05).Pearson correlation showed that there was a significant positive correlation between the expression level of lncRNA TINCR in cancer tissues and serum exosomes(P<0.05).The increased expression of lncRNA TINCR in serum exosomes and cancer tissues was associated with T stage,N stage and grade of TNBC patients(P<0.05).ROC curve results showed that the area under the curve of the expression level serum lncRNA TINCR for diagnosing TNBC was 0.772.By the follow-up time,a total of 12 TNBC patients di
