西药房药品发放差错常见原因分析及预防策略探讨

摘　　要：分析了西药房药品发放错误的原因,并探讨了相关预防策略。调取西昌市人民医院药剂科于2021年3月-2021年5月期间发生的80例西药房药晶发放记录作为对照组,发现其中出现了43起药晶分发事故。在2021年7月-2021年9月实施改进药岛分发制度后,调取80例西药房药晶发放记录作为实验组,发现只出现了19起药品分发事故。通过分析发现,西药房药品发放错误的主要原因有医师不规范书写导致药品录入信息错误、患者在没有医师的指导下自行取药、某些药晶外观相似导致混淆等。各起药品发放差错事故数值之间有明显差异(P<0.05)。研究表明,在对患者发放药品的过程中,西药房人员应严格按照药晶发放制度进行发放,并针对以往的错误案例进行深层分析,总结原因,制定更完善的药品管理制度。The causes of wrong drug distribution in western phannacy were analyzed,and relevant preventive strategies were discussed.The 80 cases of drug distribution records in the pharmacy of Xichang People's Hospital from March 2021 to May 2021 were taken as the control group,and 43 drug distribution accidents were found.After the implementation of improved drug distribution system from July 2021 to September 2021,80 drug distribution records of western phannacies were taken as the experimental group,and only 19 drug distribution accidents were found.Through analysis,it was found that the main reasons for the wrong distribution of drugs in the western pharmacy were the wrong drug entry information caused by the nonstandard writing of doctors,the patients taking drugs without the guidance of doctors,and the confusion caused by the similar appearance of some drugs.There was a significant difference in the number of drug distribution errors(P<0.05).It can be seen from this that in the process of distributing drugs to patients,western phannacy staff should distribute drugs in strict accordance with the drug distribution system,and conduct in-depth analysis of previous error cases,summarize the reasons,and develop a more complete drug management system.
