摘　　要：【目的】探讨改良冲和膏对糖尿病溃疡半阴半阳证模型大鼠创面皮肤的致敏性。【方法】将30只Wistar大鼠随机分为正常对照组、模型组、冲和膏组、改良冲和膏方Ⅰ组(冲和膏+黄芪、虎杖)、改良冲和膏方Ⅱ组(冲和膏+黄芪、白术)等5组,每组6只,连续给药14 d,观察大鼠创面致敏情况,采用苏木素-伊红(HE)染色法观察创面皮肤形态学变化,免疫组织化学法检测创面组织CD68、CD163、髓过氧化物酶(MPO)表达变化,酶联免疫吸附分析(ELISA)检测创面组织白细胞介素4(IL-4)、γ-干扰素(IFN-γ)水平的变化。【结果】(1)对用药后皮肤过敏反应程度及过敏性进行评分,各给药组均评价为“轻度致敏性”。(2)HE染色及免疫组织化学结果显示:各给药组中性粒细胞表达低于模型组;炎症初期改良冲和膏方Ⅰ组以M1型巨噬细胞表达为主,较早出现强烈的炎症反应且快速消退,而模型组到给药第14天仍以M2型巨噬细胞为主。(3)ELISA结果显示:各给药组在给药第14天创面组织IFN-γ表达水平低于模型组(P<0.01)。各给药组在给药第3、7天创面组织IL-4表达水平均处于较低水平,到给药第14天显著高于模型组(P<0.01)。【结论】冲和膏、改良冲和膏方Ⅰ、改良冲和膏方Ⅱ均具有皮肤致敏性低、对皮肤刺激性小的特点,均可降低IFN-γ的表达、增强IL-4的表达,改良冲和膏方Ⅰ效果最优。Objective To investigate the allergenicity of modified Chonghe Ointment on the traumatic skin of a rat model of diabetic ulcer with half-yin and half-yang syndrome. Methods Thirty Wistar rats were randomly divided into five groups, including normal control group, model group, Chonghe Ointment group, modified Chonghe Ointment formula Ⅰ group(Chonghe Ointment + Astragali Radix and Polygfoni Cuspidati Rhizoma et Radix),modified Chonghe Ointment formula Ⅱ group(Chonghe Ointment + Astragalus and Atractylodes),with 6 rats in each group. All the treatment groups were administered for 14 days. The rats were observed for the sensitization of the traumatic surface,and the morphological changes of the traumatic skin were observed by hematoxylin-eosin(HE)staining and immunohistochemistry. The changes in the expressions of CD68,CD163 and myeloperoxidase(MPO) in the traumatic tissue were detected by immunohistochemistry, and the changes in the levels of interleukin 4(IL-4) and γ-interferon(IFN-γ) in the traumatic tissue were detected by enzyme-linked immunosorbent assay(ELISA). Results(1)The degree of skin allergic reaction and allergenicity after drug administration were scored,and all administration groups were evaluated as“mildly allergenic”.(2)The results of HE staining and immunohistochemistry showed that the expression of neutrophils was lower in each administration group than that in the model group;M1 type macrophages were predominant in the modified Chonghe Ointment formula I group at the early stage of inflammation, and the strong inflammatory reaction appeared earlier and subsided quickly, while M2 type macrophages were still predominant in the model group until the 14th day of administration.(3)The ELISA results showed that the expression level of IFN-γ in the traumatic tissue of each administration group was lower than that of the model group on day 14(P<0.01).
