摘　　要：陆金根教授在遵循“减少患儿痛苦,保护肛门括约肌功能”的原则下,根据疾病的发展变化提出分期辨治婴幼儿肛瘘策略:早期脓肿未成,需重视日常调护,重在清热消肿;成脓期脓肿已成,需确保邪有出路;溃脓期脓肿既破,需重视局部调护,尚未成瘘者,需避免诱发因素;瘘管期瘘管已成,手术治疗为主;愈合期需促进术后恢复,加快创面愈合,并积极随访,完善术后管理。分期辨治策略可为疾病发生、演变、转归、预后各期提供不同方法和措施,实现对婴幼儿肛瘘开展多层次、多方位针对性的治疗。By following the principle of “relieving the pain of the child and protecting the function of the anal sphincter”,Professor LU Jin-Gen proposed the strategy of staging treatment for the infantile anal fistula according to the progress of the disease. In the early stage of infantile anal fistula,the abscesses have not yet formed,more attention should be paid to daily care and therapies of clearing heat and subsiding swelling should be stressed. In the pyosis phase, measures for the drainage of pus should be taken. In the post-pyosis stage, the abscess is broken and more attention should be paid to the nursing of the foci,and the triggering factors should be avoided if a fistula has not yet formed. In the fistula stage,surgery is the main therapy for the removal of the fistula. In the healing stage,measures of post-operative rehabilitation must be taken to speed up the healing of the wound,and the follow-up is needed to improve post-operative management. The strategy of staging treatment provides various methods and measures for the initiation,development,regression and prognosis of the disease,and ensures the targeted multi-dimension and multi-level treatment for the infantile anal fistula.
