摘 要:【目的】观察复方土茯苓颗粒对尿酸钠引起的急性痛风性关节炎新西兰兔的抗炎作用。【方法】将56只雄性新西兰兔随机分成7组,即正常组、模型组、秋水仙碱组、痛风舒组及复方土茯苓低、中、高剂量组,每组8只。各给药组按设定要求进行灌胃给药,正常组、模型组给予灌胃等体积的0.5%羧甲基纤维素钠(CMC-Na)溶液,每日1次,连续9 d。模型组和各给药组兔第9天给药后约1 h给予2.5%尿酸钠溶液0.4 mL和10 U/mL多黏菌素B溶液0.1 m L注入关节腔内进行急性痛风性关节炎造模。造模前(0 h)和造模后2、4、5 h测量膝关节肿胀程度,采用酶联免疫吸附分析(ELISA)检测膝关节液中白细胞计数、炎症因子白细胞介素(IL)-1β、IL-8的含量,采用苏木素-伊红(HE)染色法检测膝关节滑膜组织病理损伤程度。【结果】(1)与正常组比较,模型组膝关节出现明显肿胀,造模后2 h肿胀达到高峰(P<0.05);与模型组比较,各给药组肿胀逐渐缓解(P<0.05)。(2)与正常组比较,模型组白细胞计数显著升高(P<0.05);与模型组比较,各给药组关节液白细胞计数未见显著性差异(P>0.05),但均有降低的趋势,复方土茯苓高剂量组降低作用最明显。(3)与正常组比较,模型组IL-1β、IL-8水平显著升高(P<0.05);与模型组比较,复方土茯苓高剂量组、秋水仙碱组、痛风舒组IL-1β、IL-8水平明显降低(P<0.05)。(4)与正常组比较,模型组滑膜组织出现明显病理损伤,淋巴细胞和嗜酸性粒细胞浸润增多;与模型组比较,各给药组滑膜组织病理损伤减轻,淋巴细胞和嗜酸性粒细胞浸润减少,其中,复方土茯苓高剂量组改善效果最明显。【结论】复方土茯苓颗粒可明显减轻急性痛风性关节炎兔关节肿胀,降低关节液白细胞数量及IL-1β、IL-8水平,改善滑膜炎症。Objective To observe the anti-inflammatory effect of compound Tufuling Granules(mainly composed of Smilacis Glabrae Rhizoma)on New Zealand rabbits with acute gouty arthritis induced by sodium urate. Methods Fifty-six male New Zealand rabbits were randomly divided into 7 groups,namely,normal group,model group,colchicine group,Tongfengshu group,low-,medium-and high dose-groups of Compound Tufuling Granules,with 8 rabbits in each group. Each medication group was administered according to the set requirements,and the normal group and model group were given 0.5% sodium carboxymethyl cellulose(CMC-Na)solution with equal volume,once a day for consecutive 9 days. 0.4 m L 2.5% sodium urate solution and 0.1 mL 10 U/mL polymycin B solution were injected into the joint cavity of rabbits in the model group and the medication groups to establish a acute gouty arthritis model about one hour after the 9th day of administration. The swelling degree of the knee joint was measured before(0 h)and after(2,4,5 h)modeling. The leukocyte count,the contents of inflammatory factors interleukin(IL-1β)and IL-8 in the knee joint fluid were detected by enzyme-linked immunosorbent assay(ELISA). The pathological damage degree of the synovial tissue of the knee joint was detected by hematoxylin-eosin(HE)staining. Results(1)Compared the normal group,knee joint of model group showed obvious swelling,and the swelling reached the peak 2 hours after modeling(P<0.05). Compared with the model group,the swelling in each administration group was gradually relieved(P<0.05).(2)Compared with the normal group,white blood cell count(WBC)in model group was significantly increased(P<0.05);compared with the model group,there was no significant difference in the WBC count in the joint fluid of each administration group(P>0.05), showed a tendency to decrease,and the decrease effect of the high-dose groups of compound Tufuling Granules was the most obvious.(3)Compared with the normal group, the levels of IL-1β and IL-8 in the model group were significantly increa
