基于“肝肾同源”探讨围绝经期黄褐斑的证治

摘　　要：围绝经期黄褐斑的发病具有本虚标实的特点,肝郁血虚,肾精不足,气血失和,津液亏损是其发病之本。基于“肝肾同源”分析围绝经期黄褐斑的病症特点,可将其分为肝郁肾虚、血虚精亏、肝肾阴虚3型;针对以上3型基于“肝肾同治”的治本之法,围绝经期黄褐斑的治疗可分别采用逍遥散合二至丸以疏肝理气,益肾活血;采用归脾汤加菟丝子、鹿角霜以健脾养肝,生血调精;采用六味地黄丸补益肝肾,滋阴降火。通过补肾阴,养肝血,活血调精,使面部气血得调,黑斑得散。The pathogenesis of perimenopausal chloasma is characterized by deficiency in the origin and excess in the superficiality, and the deficiency in the origin manifests as liver depression and blood deficiency,insufficiency of kidney essence, disharmony of qi and blood, and consumption of body fluid. By following the theory of “the liver and kidney having a common source”,perimenopausal chloasma can be classified into the syndrome types of liver depression and kidney deficiency,blood deficiency and essence insufficiency,and liverkidney yin deficiency,which can be treated by Xiaoyao Powder plus Erzhi Pills for soothing liver and regulating qi, and nourishing kidney and activating blood, by Guipi Decoction with Cuscutae Semen and Cornu Cervi Degelatinatum added for invigorating spleen and nourishing liver, and promoting the production of blood and regulating essence,and by Liuwei Dihuang Pills for tonifying liver and kidney,and nourishing yin to reduce fire respectively based on the therapeutic principles of simultaneous treatment of liver and kidney. By tonifying kidney yin, nourishing liver blood, and activating blood and regulating essence, the facial qi and blood will be harmonized,and then the chloasma will be eliminated.
