摘  要:目的分析四种标本中人巨细胞病毒(HCMV)检测结果对小儿疑似HCMV感染的诊断价值。方法选择2019年8月至2021年12月在茂名市妇幼保健院就诊或收治的疑似HCMV感染小儿(0~12个月)共549例为研究对象,按年龄分为新生儿(0~28 d)26例和婴儿(29 d~12个月)523例两组。采用实时荧光定量PCR技术检测母乳和患儿尿液、肺泡灌洗液、抗凝全血的HCMV-DNA,同时用时间分辨荧光免疫分析法检测血清HCMV-IgM,并对结果进行分析。结果婴儿组HCMV阳性检出率高于新生儿组(P<0.001)。在新生儿和婴儿组中,母乳标本HCMV阳性检出率最高。但在母乳HCMV检测结果阴性病例中,存在其他标本HCMV阳性的情况,表明联合检测可相互填补漏检的病例。结论婴儿期更容易感染HCMV,联合检测有助于诊断小儿HCMV感染。若不执行联合检测时首选母乳检测。Objective To analyze the diagnostic value of detection of human cytomegalovirus(HCMV)in four types of samples for children with suspected HCMV infection.Methods A total of 549 children(0-12 months)suspected with HCMV infection who were hospitalized or treated in Maoming Maternal and Child Health Hospital from August 2019 to December 2021 were selected as the research objects.They were divided into two groups according to age:neonates(0-28 d,n=26)and infants(29 d-12 months,n=523).HCMV-DNA in human milk and the children’s urine,bronchoalveolar lavage fluid,and anticoagulant whole blood was detected by real-time fluorescent quantitative PCR technology,and serum HCMV-IgM was detected by time-resolved fluorescent immunoassay.The detection results were statistically analyzed.Results The positive rate of HCMV in infant group was higher than that in neonatal group(P<0.001).The positive rate of HCMV in breast milk samples was the highest in neonates and infants.However,in cases where the HCMV test result of human breast milk was negative,there were cases where other samples were HCMV-positive,so the joint test could fill in the missed cases.Conclusion Infants are more likely to be infected with HCMV.Combined detection is helpful for the diagnosis of HCMV infection in children.If joint testing is not performed,breast milk testing is preferred.
