摘 要:目的 分析不同乙肝病毒载量与乙肝血清标志物模式和Pre-S1抗原定性结果之间的相关性。方法选择我院2020年3-12月申请血清乙肝DNA定量检测的患者作为研究对象,收集所有的乙肝DNA定量数据,同时用ELISA法检测其乙肝血清标志物和Pre-S1抗原,将结果按照最新的指南进行分类并分析其相关性。结果本次研究的159例样本中共统计到不同乙肝血清标志物免疫模式有11种;HBV-DNA阳性率为55.97%;Pre-S1阳性率为70.44%;将乙肝五项血清标志物HBsAg、HBsAb、HBeAg、HBeAb和HBcAb分别采用①、②、③和③表示。①③模式下,各载量组阳性率:高载量组(87.10%)>中载量组(20.83%)>低载量组(2.94%)>低于检测限组(0),差异具有统计学意义(P<0.05)。①④③模式下,各载量组阳性率:低载量组(88.24%)>中载量组(79.17%)>低于检测限组(40.00%)>高载量组(9.68%),差异具有统计学意义(P<0.05)。比较各载量组Pre-S1抗原阳性率,高载量组(100%)、中载量组(100%)>低载量组(97.06%)>低于检测限组(34.29%),差异具有统计学意义(P<0.05)。比较各载量组HBeAg阳性率,高载量组(90.32%)>中载量组(20.83%)>低载量组(2.94%)>低于检测限组(0),差异具有统计学意义(P<0.05)。比较各载量组HBcAb阳性率,中载量组(100%)、低载量组(100%)>低于检测限组(84.29%)>高载量组(96.77%),差异具有统计学意义(P<0.05)。HBV-DNA载量均值与HBeAg阳性率之间存在显著相关性(r=1.000,P=0.000),HBV-DNA载量均值与Pre-S1阳性率之间相关性不显著(r=0.211,P=0.789),HBV-DNA载量均值与HBcAb阳性率之间相关性不显著(r=0.316,P=0.684)。结论 不同乙肝病毒载量与乙肝血清标志物中的HbeAg阳性率存在显著相关性,而与Pre-S1抗原定性结果的相关性不显著;临床应多维度、多方法结合评估患者病情,达到有效治疗的目的。Objective To analyze the correlation between different HBV loads with HBV serum marker pattern and Pre-S1 antigen qualitative results.Methods HBV DNA data,HBV serum markers and Pre-S1 antigen collected from March to December 2020,were selected as the research objects.According to the latest guidelines,all results were classified and analyzed for difference.ResultsAccording to HBV serum markers,159 samples can be divided into 11 kinds of immune patterns.HBV-DNA positive rate was 55.97%.Pre-S1 antigen positive rate was 70.44%.HBsAg,HBsAb,HBeAg,HBeAb and HBcAb were expressed by,@,,@and,respectively.In pattern@,the positive rate of each load group was as fllows:high load group(87.10%)>medium load group(20.83%)>low load group(2.94%)>lower than detection limit group(0),the difference was statistically significant among the four groups(P<0.05).In pattern@,the positive rate of each load group was as fllows:low load group(88.24%)>medium load group(79.17%)>lower detection limit group(40.00%)>high load group(9.68%),the difference among the four groups was statistically significant(P<0.05).The positive rate of pre-S1 antigen in all groups:high load group(100%)、medium load group(100%)>low load group(97.06%)>lower than the detection limit group(34.29%),the difference was statistically significant among the four groups(P<0.05).The positive rate of HBeAg in all groups:high load group(90.32%)>medium load group(20.83%)>low load group(2.94%)>lower than the detection limit group(0),the difference was statistically significant among the four groups(P<0.05).The positive rate of HBcAb in all groups:medium load group(100%)、low load group(100%)>high load group(96.77%)>lower than that in detection limit group(84.29%),the difference was statistically significant among the four groups(P<0.05).There was a significant correlation between HBV-DNA mean load and HBeAg positive rate(r=1.000,P-0.000).There was no significant correlation between HBV-DNA mean load and pre-S1 positive rate(r=0.211,P-0.789),There was no significant corelation betw
