摘  要:目的 探究周围型非小细胞肺癌(NSCLC)表皮生长因子受体(EGFR)基因突变与超声征象的相关性。方法 回顾性分析经穿刺活检行病理确诊为NSCLC患者80例。先行超声造影(CEUS)检查,随后超声引导下穿刺活检取材行EGFR基因突变检测,根据EGFR基因突变与否分为阳性组和阴性组,对比两组临床病理、常规超声及CEUS特征,分析周围型NSCLC EGFR基因突变的独立相关因素。结果 与阴性组相比,CEUS呈高增强、均匀增强、女性、肺腺癌上多见于EGFR基因突变阳性组,差异有统计学意义(P<0.05)。二元Logistic回归分析显示,4个因素均为周围型NSCLC EGFR基因突变的独立相关因素。结论 周围型NSCLC中CEUS呈高增强、均匀增强、女性、肺腺癌与EGFR基因突变有相关性,有助于EGFR基因突变状态的辅助预测,为治疗决策提供一定意义的指导。Objective To explore the correlation between EGFR mutation and ultrasound signs in NSCLC. Methods A retrospective analysis was performed on 80 patients with NSCLC who were pathologically confirmed by biopsy. All patients were divided into EGFR positive group and EGFR negitive group and performed CEUS before the treatment. General data, conventional ultrasound and CEUS characteristics of the two groups were compared and analyzed. Results The proportion of CEUS with high enhancement, uniform enhancement, female and lung adenocarcinoma in the positive group was higher than that in the negitive group, they were associated with EGFR gene mutation. Conclusions CEUS with high enhancement, uniform enhancement, female sex and lung adenocarcinoma are correlated with EGFR mutation. It is helpful for auxiliary prediction of EGFR mutation status and provides certain guiding significance for treatment.
