摘  要:目的 探讨卵巢-附件超声报告和数据风险分层系统(O-RADS)在诊断附件区良恶性肿瘤中的应用。方法 选取接受妇科超声检查的附件区肿块共计110个,且均经手术证实,采用回顾性分析,将图像进行O-RADS分类,以病理为金标准,创建受试者工作特征(ROC)曲线,计算O-RADS诊断附件区肿瘤良恶性的效能。结果 (1)O-RADS 2、3、4、5类中附件区恶性肿瘤诊断符合率分别为0、8.33%、27.27%、78.38%;(2)O-RADS诊断附件肿瘤性质时,曲线下面积(AUC)为0.909,O-RADS 5类为最佳截断值,此时灵敏度为80.56%、特异度为89.19%、准确度为86.36%;(3)不同医师对于O-RADS分类表现较好一致性,组内相关系数为0.975,P<0.001。结论 O-RADS在附件区良恶性肿瘤诊断中有较好诊断效能。Objective To explore the application of Ovarian-Adnexal Ultrasound Reporting and Data System(O-RADS) in the diagnosis of benign and malignant tumors in the adnexal area. Methods 110 masses in the adnexal area that underwent gynecological ultrasound were collected, and confirmed by surgery. Images were retrospectively analyzed using O-RADS classification, and the receiver operator characteristic(ROC) curve with pathology as the gold standard were created to calculate the efficacy of O-RADS in the diagnosis of benign and malignant adnexal tumors. Results(1)The coincidence rates of O-RADS 2,3,4,and 5 in adnexal malignancy were 0,8.33%,27.27% and 78.38%, respectively.(2) The area under the curve of ROC of O-RADS in ovarian-adnexal tumor diagnosis was 0.909,the cutoff value was O-RADS 5,and the sensitivity was 80.56%, specificity was 89.19%, accuracy was 86.36%.(3)The O-RADS consistency performed well among the different physicians, with an intraclass correlation coefficient of 0.975,P<0.001. Conclusions O-RADS has a good diagnostic efficacy in the diagnosis of adnexal tumors.
