摘  要:目的 了解浙西南妊娠女性生殖道病原体感染特点,为防控和诊疗提供科学依据。方法 收集2020年1月—2021年12月浙西南地区某院接诊的341例孕妇生殖道分泌物,检测白带常规、培养及药敏。结果 本地区妊娠女性生殖道病原体检出率为56.01%,细菌、淋球菌、支原体感染阳性占比分别为49.74%、0.52%、79.06%,混合感染以支原体和细菌双重感染为主。细菌主要为假丝酵母菌属、大肠埃希菌和无乳链球菌。大肠埃希菌对复方磺胺耐药率(61.54%)最高,无乳链球菌对四环素(100.00%)、克林霉素(91.67%)和红霉素(83.33%)耐药率较高,假丝酵母菌属无耐药菌检出。支原体以UU为主,其次为UU/MH双重感染,MH感染最少。UU耐药率低,而MH耐药率高,UU/MH有广谱耐药。阴道清洁度异常组感染率高于正常组(χ^(2)=9.47,P=0.002)。结论 本地区妊娠女性生殖道感染以支原体为主,不同感染耐药差异大,治疗应根据药敏结果选择适宜的药物。Objective This paper aims to understand the characteristics of genital tract pathogen infection in pregnant women in Southwestern Zhejiang so as to provide scientific basis for prevention, control, diagnosis and treatment of it. Methods Leukorrhea samples of 341 pregnant women from a hospital of Southwestern Zhejiang from January 2020 to December 2021 were collected for the routine test and culturing of leucorrhea and drug sensitivity test. Results The infection rate of vaginal pathogens for pregnant women was 56.01%. Among them, the infection proportions of bacteria, gonococcus and mycoplasma were 49.74%, 0.52% and 79.06%, respectively. Dual infection of bacteria and mycoplasma was predominant in mixture infection. Candida, Escherichia coli and Streptococcus agalactiae were main strains for bacterial infection. The drug resistance rate of Escherichia coli to compound sulfamethoxazole was the highest(61.54%);Streptococcus agalactiae had rather high drug-resistance rates to tetracycline(100.00%), clindamycin(91.67%) and erythromycin(83.33%);no drug-resistance was found for Candida. For mycoplasma infection, UU was the most popular strain with lowest drug-resistance, and UU/MH dual infection was the second with extensively drug resistant, and MH was the last one with the highest drug-resistance. The infection rate of vaginal pathogens in abnormal vaginal clearing degree group was significantly higher than normal group(χ^(2)=9.47, P=0.002). Conclusion Mycoplasma is the main vaginal pathogen for pregnant women in the southwest region of Zhejiang Province and the drug-resistances for different vaginal pathogens vary greatly. Clinician should be more cautious for anti-infective drugs according to drug-resistance test.
