微小RNA-125a和癌胚抗原在恶性胸腔积液诊断中的应用价值

摘　　要：目的 探讨微小RNA(miRNA)-125a和癌胚抗原(CEA)的检测在良恶性胸腔积液鉴别诊断中的应用价值。方法 收集良恶性胸腔积液患者各35例,采用实时荧光定量PCR测定miRNA-125a的相对表达量,全自动免疫分析仪测定血清CEA水平,应用ROC曲线评估miRNA-125a和CEA的诊断性能。结果 恶性胸腔积液组患者miRNA-125a相对表达量低于良性胸腔积液组,差异有统计学意义(P<0.05);恶性胸腔积液组患者CEA浓度高于良性胸腔积液组,差异有统计学意义(P<0.01)。ROC曲线分析,miRNA-125a和CEA诊断恶性胸腔积液曲线下面积(AUC)分别为0.687和0.822。结论 miRNA-125a在恶性胸腔积液中呈低表达,可能具有抑制相关肿瘤转移至胸腔的功能。胸腔积液中miRNA-125a和CEA检测对恶性胸腔积液的诊断具有一定的辅助性价值。Objective This paper aims to explore the application value of miRNA-125a and carcinoembryonic antigen(CEA) in differential diagnosis of benign and malignant pleural effusions. Methods Thirty-five patients with benign and malignant pleural effusion were collected. The relative expression level of miRNA-125a was determined by real-time fluorescence quantitative PCR, serum CEA level was determined by automatic immunoanalyzer, and the diagnostic performance of miRNA-125a and CEA was evaluated by ROC curve. Results The relative expression of miRNA-125a in the malignant pleural effusion group was lower than that in the benign pleural effusion group, and the difference was statistically significant(P<0.05). The concentration of CEA in the malignant pleural effusion group was higher than that in the benign pleural effusion group, and the difference was statistically significant(P<0.01). ROC curve analysis showed that the area under the curve(AUC) of miRNA-34a and CEA in the diagnosis of malignant pleural effusion were 0.687 and 0.822, respectively. Conclusion The low expression of miRNA-125a in malignant pleural effusion may inhibit the metastasis of related tumors to the pleural cavity. The detection of miRNA-125a and CEA in pleural effusion has certain auxiliary value for the diagnosis of malignant pleural effusion.
