摘  要:目的 检测老年慢性心力衰竭患者血浆微小RNA-487b(miR-487b)和微小RNA-154(miR-154)的表达情况,分析其在老年慢性心力衰竭发病过程中的作用。方法 选取2019年2月—2022年4月在浙江省台州医院确诊并住院治疗的老年慢性心力衰竭患者130例为慢性心力衰竭组,根据心功能分级分为Ⅱ级35例、Ⅲ级54例、Ⅳ级41例;选取在医院进行体检的健康老年人130例为对照组。采用实时荧光定量PCR法检测各组血浆miR-487b和miR-154表达水平;彩色多普勒超声心动图仪测定各组的心功能指标[左室舒张末期内径(LVEDD)、左室射血分数(LVEF)];Pearson法分析老年慢性心力衰竭患者血浆miR-487b、miR-154、心功能指标的相关性;logistic回归分析影响老年慢性心力衰竭发病的因素;受试者工作特征曲线评估血浆miR-487b、miR-154对心功能分级Ⅳ级老年慢性心力衰竭患者的诊断价值。结果 与对照组比较,慢性心力衰竭组血浆miR-487b、miR-154表达水平及LVEF降低,LVEDD升高(P<0.05);Ⅱ级、Ⅲ级、Ⅳ级老年慢性心力衰竭患者血浆miR-487b、miR-154表达水平及LVEF依次降低,LVEDD依次升高(P<0.05);老年慢性心力衰竭患者血浆miR-487b与miR-154成正相关(P<0.05);血浆miR-487b、miR-154与LVEDD成负相关(P<0.05),与LVEF成正相关(P<0.05);LVEDD是老年慢性心力衰竭发病的危险因素,miR-487b、miR-154、LVEF是老年慢性心力衰竭发病的保护因素(P<0.05);血浆miR-487b、miR-154、二者联合诊断心功能分级Ⅳ级老年慢性心力衰竭患者的曲线下面积(AUC)分别为0.789、0.727、0.907,二者联合的AUC优于miR-487b、miR-154各自单独诊断(P<0.05)。结论 老年慢性心力衰竭患者血浆miR-487b与miR-154呈低表达,二者与心功能密切相关,均可作为评估患者病情的指标。Objective This paper aims to detect the expression of microRNA-487b(miR-487b) and microRNA-154(miR-154) in the plasma of elderly patients with chronic heart failure, and analyze their role in the pathogenesis of elderly chronic heart failure. Methods From February 2019 to April 2022, 130 elderly patients with chronic heart failure diagnosed and hospitalized in Taizhou Hospital were selected as the chronic heart failure group, according to the cardiac function classification, there were 35 cases of grade Ⅱ, 54 cases of grade Ⅲ and 41 cases of grade Ⅳ;130 healthy elderly people who underwent physical examination in the hospital were selected as the control group. The levels of plasma miR-487b and miR-154 in each groups were detected by real-time fluorescent quantitative PCR;the cardiac function [left ventricular end diastolic diameter(LVEDD) and left ventricular ejection fraction(LVEF)] were measured by color Doppler echocardiography in each groups. Pearson method was used to analyze the correlation between plasma miR-487b, miR-154 and cardiac function indexes in elderly patients with chronic heart failure. Logistic regression was adopted to analyze the factors affecting the incidence of chronic heart failure in the elderly;the receiver operating characteristic curve was used to evaluate the diagnostic value of plasma miR-487b and miR-154 in elderly patients with chronic heart failure with cardiac function grade Ⅳ. Results Compared with the control group, the levels of miR-487b, miR-154 and LVEF in chronic heart failure group decreased, and LVEDD increased(P<0.05);the levels of miR-487b, miR-154 and LVEF in plasma of grade Ⅱ, grade Ⅲ and grade Ⅳ elderly patients with chronic heart failure decreased in turn, and LVEDD increased in turn(P<0.05);there was a positive correlation betweenplasma miR-487b and miR-154 in elderly patients with chronic heart failure(P<0.05);plasma miR-487b and miR-154 were negatively correlated with LVEDD(P<0.05), and positively correlated with LVEF(P<0.05);LVEDD was the risk fa
